Usual Adult Dose for Cystitis
500 mg orally twice a day for 3 to 7 days, depending on the nature and severity of the infection.
Usual Pediatric Dose for Cystitis
Child > 12 years: 500 mg orally twice a day for 3 to 7 days, depending on the nature and severity of the infection.
Renal Dose Adjustments
CrCl < 20 mL/min: 250 mg orally once a day for 3 to 7 days.
CrCl 20 to 49 mL/min: 250 mg orally twice a day for 3 to 7 days.
CrCl 50 to 80 mL/min: 250 mg orally 3 times a day for 3 to 7 days.

Other Comments
Advise patients on cinoxacin therapy to remain well hydrated to prevent crystalluria.
