Cefonicid Dosage
Applies to the following strengths: 1 g
Usual Adult Dose for:
- Pneumonia
- Urinary Tract Infection
- Surgical Prophylaxis
- Cesarean Section
- Joint Infection
- Osteomyelitis
- Skin and Structure Infection
- Sepsis
Additional dosage information:
Usual Adult Dose for Pneumonia
Mild to moderate infections: 1 g IV or intramuscularly every 24 hours.
Severe or life-threatening infections: 2 g IV every 24 hours.
Usual Adult Dose for Urinary Tract Infection
Uncomplicated: 0.5 g IV or intramuscularly once every 24 hours.
Usual Adult Dose for Surgical Prophylaxis
Preoperative: 1 g IV or intramuscularly 60 minutes prior to procedure.
Intraoperative: not necessary.
Postoperative: 1 g IV or intramuscularly may be administered for 2 additional days in patients undergoing prosthetic arthroplasty or open heart surgery.
Usual Adult Dose for Cesarean Section
1 g IV or intramuscularly after the umbilical cord has been clamped.
Usual Adult Dose for Joint Infection
Mild to moderate infections: 1 g IV or intramuscularly every 24 hours. Severe or Life-threatening infections: 2 g IV every 24 hours.
Usual Adult Dose for Osteomyelitis
Mild to moderate infections: 1 g IV or intramuscularly every 24 hours. Severe or Life-threatening infections: 2 g IV every 24 hours.
Usual Adult Dose for Skin and Structure Infection
Mild to moderate infections: 1 g IV or intramuscularly every 24 hours. Severe or Life-threatening infections: 2 g IV every 24 hours.
Usual Adult Dose for Sepsis
Mild to moderate infections: 1 g IV or intramuscularly every 24 hours. Severe or Life-threatening infections: 2 g IV every 24 hours.
Renal Dose Adjustments
CrCl < 5 mL/min:
Mild to moderate infections: Loading dose of 7.5 mg/kg followed by 3 mg/kg IV or intramuscularly every 3 to 5 days.
Severe infections: Loading dose of 7.5 mg/kg followed by 4 mg/kg IV every 3 to 5 days.
CrCl 5 to 9 mL/min:
Mild to moderate infections: Loading dose of 7.5 mg/kg followed by 4 mg/kg IV or intramuscularly every 3 to 5 days.
Severe infections: 15 mg/kg IV every 3 to 5 days.
CrCl 10 to 19 mL/min:
Mild to moderate infections: Loading dose of 7.5 mg/kg followed by 4 mg/kg IV or intramuscularly every 48 hours.
Severe infections: 15 mg/kg IV every 48 hours.
CrCl 20 to 39 mL/min:
Mild to moderate infections: Loading dose of 7.5 mg/kg followed by 4 mg/kg IV or intramuscularly every 24 hours.
Severe infections: 15 mg/kg IV every 24 hours.
CrCl 40 to 9 mL/min:
Mild to moderate infections: 8 mg/kg IV or intramuscularly every 24 hours.
Severe infections: 20 mg/kg IV every 24 hours.
CrCl 60 to 79 mL/min:
Mild to moderate infections: 10 mg/kg IV every 24 hours
Severe infections: 25 mg/kg IV every 24 hours.
Liver Dose Adjustments
No adjustment recommended
Dialysis
It is not necessary to administer additional dosage following dialysis.
Other Comments
When administering 2 g intramuscular doses once daily, 1/2 the dose should be administered in different large muscle masses.
