Alosetron (Includes Lotronex) ↔ gastrointestinal complications

Severe Potential Hazard, Moderate plausibility. Applies to: Gastrointestinal Obstruction, Gastrointestinal Perforation, Constipation

Alosetron is contraindicated in patients with a history of severe bowel complications. Patients with serious complications of constipation, including history of chronic or severe constipation or sequelae from constipation; intestinal obstruction, stricture, toxic megacolon, gastrointestinal perforation, and/or adhesions; ischemic colitis; obstruction, ileus, impaction, toxic megacolon, secondary bowel ischemia, and impaired intestinal circulation should not use alosetron. Care should be exercised when using this agent in patients at risk.

Major

Alosetron (Includes Lotronex) ↔ hepatic impairment

Severe Potential Hazard, Moderate plausibility. Applies to: Liver Disease

Alosetron is extensively metabolized by the liver. The use of alosetron is contraindicated in patients with severe hepatic impairment and it should be used with caution in patients with mild or moderate hepatic impairment. Close monitoring is recommended.

Major

Alosetron (Includes Lotronex) ↔ thrombotic disorders

Severe Potential Hazard, Moderate plausibility. Applies to: Thrombotic/Thromboembolic Disorder

The use of alosetron is contraindicated in patients with coagulation abnormalities such as thrombophlebitis, or hypercoagulable state.

Moderate

Alosetron (Includes Lotronex) ↔ renal impairment

Moderate Potential Hazard, Moderate plausibility. Applies to: Renal Dysfunction

Renal impairment has no effect on the renal elimination of alosetron due to the minor contribution of this pathway to elimination. Care should be exercised as the effect of renal impairment on metabolite pharmacokinetics and the effect of end-stage renal disease have not been assessed.
