Usual Adult Dose for Hypertension
25 to 50 mg orally twice a day after meals.
Usual Adult Dose for Edema
Initial dose: 25 to 100 mg orally twice a day after meals.
Maintenance dose: 50 to 150 mg/day orally in 2 to 3 divided doses.
Renal Dose Adjustments
CrCl < 25 mL/min: Not recommended.
CrCl 25-80 mL/min: Initial dose: 25 mg orally twice a day after meals. Maintenance dose: 50 to 100 mg/day orally in a single or 2 divided doses.

The maximum recommended daily dose is 200 mg orally in 2 to 4 divided doses
Periodic monitoring of electrolytes is recommended, particularly in elderly patients and in patients receiving a high dose.
