Usual Adult Dose for Hypertension
Initial dose: 1 tablet orally once a day.
Maintenance dose: 1 to 2 tablets/day in 1 to 2 divided doses. Dosage should be determined by individual titration.
Dosage regimens that exceed 0.25 mg of reserpine per day are not recommended.
Renal Dose Adjustments
Hydrochlorothiazide is generally not effective in patients with creatine clearance less than 25 mL/min. Use hydrochlorothiazide with caution in severe renal disease.
Liver Dose Adjustments
Thiazides should be used with caution in patients with impaired liver function.
Dose Adjustments
Daily reserpine and hydrochlorothiazide doses greater than 0.25 and 50 mg, respectively, are generally not recommended. If there is an inadequate antihypertensive effect with two and one-half tablets, additional hydralazine or alternative antihypertensive therapy should be considered.
Dialysis
Dialysis may not be effective because of its plasma protein binding.
