There are 3 disease interactions with Adempas (riociguat):

Moderate

Riociguat (Includes Adempas) ↔ Hypotension

Moderate Potential Hazard, Moderate plausibility

Applies to: Hypotension, Hypertension

Riociguat reduces blood pressure. There is a potential for symptomatic hypotension or ischemia in patients with hypovolemia, severe left ventricular outflow obstruction, resting hypotension, autonomic dysfunction, or concomitant treatment with antihypertensive agents. Caution is advised when using riociguat in these patients.

Moderate

Riociguat (Includes Adempas) ↔ Liver Impairment

Moderate Potential Hazard, Moderate plausibility

Applies to: Liver Disease

The safety and efficacy of riociguat have not been demonstrated in patients with severe hepatic impairment (Child-Pugh C). Care should be exercised when using riociguat in these patients.

Moderate

Riociguat (Includes Adempas) ↔ Renal Impairment

Moderate Potential Hazard, Moderate plausibility

Applies to: Renal Dysfunction

The safety and efficacy of riociguat have not been demonstrated in patients with creatinine clearance < 15 mL/min or on dialysis. Care should be exercised when using riociguat in these patients.

Drug Interaction Classification

The classifications below are a general guideline only. It is difficult to determine the relevance of a particular drug interaction to any individual given the large number of variables.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
