Usual Adult Dose for Hyperkalemia

-Initial dose: 8.4 g orally once a day
-Maintenance dose: The dose should be adjusted based on the serum potassium level and the desired target range (up to the maximum dose of 25.2 g orally once a day). The dose can be up-titrated based on the serum potassium level at 1 week or longer intervals, in increments of 8.4 g.

Comments:
-This drug should be administered with food.
-This drug should not be heated (e.g., microwave) or added to heated foods or liquids.
-This drug is not intended to be taken in its dry form.
-This drug should be administered at least 6 hours before or 6 hours after other oral medications.
-Serum potassium levels should be monitored.

-Use: For the treatment of hyperkalemia
-Limitation of Use: This drug should not be used as an emergency treatment for life-threatening hyperkalemia because of its delayed onset of action.

Precautions

US BOXED WARNINGS:
This drug binds to many orally administered medications, which could decrease their absorption and reduce their effectiveness. Other oral medications should be administered at least 6 hours before or 6 hours after this drug. Choose this drug or the other oral medication if adequate dosing separation is not possible.

Safety and efficacy have not been established in patients younger than 18 years.

Storage requirements:
-Store in the refrigerator at 2C to 8C (36F to 46F).
-If stored at room temperature (25C [77F]), this drug should be used within 3 months of being taken out of the refrigerator.
-For either storage condition, this drug should not be used after the expiration date printed on the packet.
-Avoid exposure to heat above 40C (104F).

Reconstitution/preparation techniques:
Prepare each dose immediately prior to administration following the steps below:
-Step 1: Add 30 mL of water to an empty glass or cup.
-Step 2: Empty the entire contents of the packet(s) into the glass or cup.
-Step 3: Stir the mixture thoroughly.
-Step 4: Add an additional 60 mL of water to the glass or cup containing the mixture.
-Step 5: Stir the mixture thoroughly; the powder will not dissolve and the mixture will look cloudy.
-Step 6: Drink the mixture immediately. If some powder remains in the glass after drinking, add more water, stir and drink immediately. Repeat as needed to ensure the entire dose is administered.

Patient advice:
-Advise the patient to read the FDA-approved patient labeling (Medication Guide).
-Inform patients that this drug should not be heated (e.g., microwaved) or added to heated foods or liquids and should not be taken in its dry form.
-Inform patients to take this drug as directed with food and adhere to their prescribed diets.
-Instruct patients to prepare each dose separately using the preparation instructions provided in the FDA-approved patient labeling (Medication Guide).
-Instruct patients who are taking other oral medication(s) to separate the dosing of this drug by at least 6 hours (before or after).
