Drotrecogin alfa was voluntarily withdrawn from the market by the manufacturer in October, 2011 due to failure to show a survival benefit for patients with severe sepsis and septic shock. The following dosage information applies to when the drug was available in the U.S.

Liver Dose Adjustments
Use with caution in patients with chronic severe hepatic disease due to increased risk of bleeding.
Dose Adjustments
If the infusion is interrupted, restart at 24 mcg/kg/hr. Do not bolus or increase dose.
Precautions
Stop infusion if bleeding occurs.
Drotrecogin alfa may prolong APTT. Prothrombin time (PT) is recommended for monitoring the status of coagulopathy.
There are no data on the safety and efficacy of administering more than 1 course of drotrecogin alfa to septic patients.
Efficacy has not been established in septic patients with a lower risk of death (APACHE II score<25).
Safety and efficacy have not been established in children <18 years.

The infusion should be stopped if clinically significant bleeding occurs. Reinitiation of drotrecogin alfa may be considered after hemostasis is achieved.
Drotrecogin alfa should be discontinued 2 hours before surgery or procedures with a risk of bleeding. Reinitiation may be considered 12 hours after the procedure if hemostasis has been achieved.
