ADJUST DOSE: The use of oxygen in patients undergoing bleomycin treatment may increase the severity of bleomycin-associated pulmonary toxicity. Severe and fatal respiratory distress syndrome, pneumonitis, and pulmonary fibrosis have been reported. The interaction may be dose-dependent. Oxygen concentrations higher then 30% may increase the risk of pulmonary toxicity. A study reported that the use of lower oxygen concentrations (22% to 30%) decreased the incidence of respiratory distress syndrome during surgery. The mechanism is unknown.
MANAGEMENT: The manufacturer considers the use of pulmonary function tests with 100% oxygen to be contraindicated in bleomycin-treated patients, and recommends the use of pulmonary function tests with less than 21% oxygen. During general anesthesia, it is recommended that bleomycin-treated patients receive oxygen concentrations less than 30%.
