Mafenide topical (applies to Sulfamylon) acid/base balance
Moderate Potential Hazard, High plausibility. Applicable conditions: Pulmonary Impairment, Renal Dysfunction
Topically applied mafenide is absorbed and rapidly converted to a metabolite that is eliminated by the kidney. Mafenide and its metabolite inhibit carbonic anhydrase and can induce metabolic acidosis, particularly if drug levels are high or if the compensatory hyperventilation mechanism is impaired. Therapy with mafenide should be administered cautiously in patients with extensive second-degree or partial thickness burns and in patients with pulmonary or renal dysfunction. Acid-base balance should be monitored closely.
Mafenide topical (applies to Sulfamylon) hemolytic anemia/DIC
Moderate Potential Hazard, High plausibility. Applicable conditions: G-6-PD Deficiency
Hemolytic anemia with disseminated intravascular coagulation associated with a glucose-6 phosphate deficiency (G6PD) has occurred during therapy with mafenide topical. Therapy with mafenide topical should be applied cautiously in patients with G6PD deficiency.
