As a student pharmacist, I would interpret this as
Day 1: 45mg, Day 2: 45mg,
Day 3: 40mg, Day 4:40mg,
Day 5: 35mg, Day 6, 40mg
And so on. You can always contact your doctor or pharmacy if you are unsure of the dosage
