Elligibility requirements:

  1. Must have no prescription coverage
  2. At or below 400% of FPL
  3. Medically appropriate condition/diagnosis
  4. The patient must be a US citizen or legal resident.
  5. The manufacturer offering this program does not charge patients or their agents for applying to the program, or for any medication eligible patients receive from the program. Applicants who choose to use the services of a commercial advocacy service for assistance with the application process and/or with communicating with the program may have to supply additional documentation.
