Interactions between Pramlintide and VESIcare
1 drug interaction and 10 additional warnings found for the following 2 drugs:
- pramlintide
- VESIcare (solifenacin)
Drug interactions
Moderate drug interactions
Moderate:
solifenacin pramlintide
Applies to: VESIcare (solifenacin) and pramlintide
Generally avoid
The use of pramlintide in combination with drugs that alter gastrointestinal motility (e.g., loperamide; anticholinergic agents) or drugs that slow the intestinal absorption of nutrients (e.g., alpha-glucosidase inhibitors) has not been studied. Because pramlintide slows gastric emptying, the potential for a pharmacodynamic interaction is possible.
Management
Due to its effect on gastric emptying, pramlintide therapy should not be considered for patients taking drugs that alter gastrointestinal motility or drugs that slow the intestinal absorption of nutrients.

Food/lifestyle interactions
Moderate food/lifestyle interactions
Moderate:
pramlintide Food/Lifestyle
Applies to: pramlintide
Adjust dosing interval
Pramlintide slows gastric emptying and may delay the absorption of concomitantly administered oral medications. In a pharmacokinetic study of 24 patients with type 2 diabetes, coadministration with pramlintide (120 mcg) decreased the peak plasma concentration (Cmax) of acetaminophen (1000 mg) by 29% and increased its time to peak plasma concentration (Tmax) based on the time of acetaminophen administration relative to pramlintide injection. Pramlintide significantly increased acetaminophen Tmax (range 48 to 72 minutes) when acetaminophen was administered simultaneously with or up to 2 hours following pramlintide injection, but it had negligible effect when acetaminophen was administered 1 to 2 hours before pramlintide injection.
Management
When rapid onset of a concomitantly administered oral medication is critical to its effectiveness, the medication should be administered at least 1 hour before or 2 hours after pramlintide injection.

Disease interactions
9 potential interactions Applies to: 9 conditions, including Gastroparesis, Dementia, Urinary Retention, Liver Disease, and Long QT Syndrome.
Central Nervous System Disorders
Moderate:
VESIcare
The solifenacin in VESIcare has a moderate potential interaction with Central Nervous System Disorders.
Solifenacin is associated with anticholinergic central nervous system (CNS) effects. Patients should be monitored for signs of anticholinergic CNS effects, particularly after beginning treatment or increasing the dose. If a patient experiences anticholinergic CNS effects, dose reduction or drug discontinuation should be considered. Solifenacin should be used with caution in patients with Parkinson's disease and in patients with preexisting dementia treated with cholinesterase inhibitors due to the risk of aggravation of symptoms.
Dementia
Moderate:
VESIcare
The solifenacin in VESIcare has a moderate potential interaction with Dementia.
Gastrointestinal Obstruction
Major:
VESIcare
The solifenacin in VESIcare has a major potential interaction with Gastrointestinal Obstruction.
The use of solifenacin is contraindicated in patients with gastric retention. Caution is advised even when using this agent in patients with decreased gastrointestinal motility.
Gastroparesis
Major:
pramlintide
Pramlintide has a major potential interaction with Gastroparesis.
The use of pramlintide is contraindicated in patients with a confirmed diagnosis of gastroparesis as it slows gastric emptying.
Glaucoma, Narrow Angle
Major:
VESIcare
The solifenacin in VESIcare has a major potential interaction with Glaucoma, Narrow Angle.
The use of solifenacin is contraindicated in patients with uncontrolled narrow-angle glaucoma. Caution is advised even when using this agent in patients with narrow-angle glaucoma.
Liver Disease
Major:
VESIcare
The solifenacin in VESIcare has a major potential interaction with Liver Disease.
Solifenacin should be used with caution in patients with reduced hepatic function. Increase in half-life and AUC have been demonstrated in patients with reduce hepatic impairment. Caution should be taken when prescribing this agent to patients with moderate hepatic impairment and the use of doses greater than 5 mg in these patients is not recommended by the manufacturer. Solifenacin is not recommended in patients with severe hepatic impairment. .
Long QT Syndrome
Moderate:
VESIcare
The solifenacin in VESIcare has a moderate potential interaction with Long QT Syndrome.
Dose dependent prolongation in QT interval has been observed with the use of solifenacin. This drug should be used with caution in patients with a known history of QT prolongation or patients who are taking medications known to prolong the QT interval.
Renal Dysfunction
Major:
VESIcare
The solifenacin in VESIcare has a major potential interaction with Renal Dysfunction.
Solifenacin should be used with caution in patients with renal impairment. Increase in half-life and AUC have been demonstrated in patients with severe renal impairment. Caution should be taken when prescribing this agent to patients with renal impairment. Doses greater than 5 mg in patients with severe renal impairment is not recommended by the manufacturer.
Urinary Retention
Major:
VESIcare
The solifenacin in VESIcare has a major potential interaction with Urinary Retention.
The use of solifenacin is contraindicated in patients with urinary retention. Caution is advised even when using this agent in patients with clinically significant bladder outflow obstruction.
