"The box says: Prep. H PE GEL 'Cooling effect.' It works. It 'cools' after it burns for 5 minutes! I am not a doctor, but I think it's one of the non-medical ingredients. Other witch hazel products don't burn. Honestly, the rectal tube that comes with it is the best around! Ha-ha... If you are a large 6' man, there will be no problem. It has holes in the sides. I tried taping the 'higher' holes so it came out the tip more because it's pretty long. But good for internal piles. I use my finger very carefully with 2 dabs on my finger. It's gross to me. I am in the loo and next to a sink. I wash my hands. It's good for when I know I will be sitting for a couple of hours. Chronic piles. It does burn quite a bit. I don't know why. Recently, the piles have been so painful, I didn't notice the burn... But it does not cool right away, and the tip might be too long for you."

"I've used so many things. It sucks! It's gross! This stuff is OK. It's probably the best OTC for piles. From everything I've used, I had to get prescription steroids. Anyway, the tips on everything are pretty bad. It's hard to bend over and use them. This long, thin design with side holes is good if you're a large man. Smaller people will have issues, I did. Used my finger carefully. It burns a bit before it 'cools.' Ugh, at least I was next to the sink. It works well. I prefer the gel over the cream!"
