I have been suffering from vaginal atrophy for few years now and its better when I use Vagifem but problems start again when I stop the treatment after around a month: dryness, burning sensation, wanting to urge to pee. Does it mean that I need to use this treatment all the time? I have tried to use hormone free pessaries With HA but they are big and it hurts when i insert them . I am also worried to go to the GP for a smear in case it hurts too much. Any suggestions?

Yes, you need to use Vagifem all the time. Unfortunately stopping will send you right back to having all the symptoms. Be sure your GP uses the smallest speculum when you have your exam, and that it is very well lubricated. However if the pessaries hurt you the exam probably will too.
