I was on 20 mg of fluoxetine for about 8 months. Then doctor increased my medication to 40mg. It’s been about a week and my anxiety and panic attacks seem to be worse. Is that possible ? Should an increase make my anxiety better since I had an increase in medication? I wake up in the early morning with heart palpitations and feel very uncontrolled.. please help...

Thank you for your response as I’m also concerned about the increased nervousness as I’m just on day 5 of starting 10 mg ... it does get better though! Sleep too ?

"Side effects have been reported to be generally mild and transient. They are most common during the first 2 weeks of treatment and decrease in intensity and frequency with continued treatment. They generally do not lead to treatment cessation."
"Very common (10% or more): Insomnia (up to 33%), anxiety (up to 15%), nervousness (up to 14%)"
"Very common (10% or more): Headache (up to 21%), somnolence (up to 17%), tremor (up to 13%), dizziness (up to 11%)"
Jacki, as you can see these side effects are very common and generally resolve within just a few days to a couple of weeks... and they lessen every day. Prozac can cause some insomnia ~OR~ increased sleepiness... it will go away as your metabolism adjusts to the medication.
Stay positive and try to be patient. All will be good soon!

One more question ... I’m only on day 8 and the last couple days I started getting a random twitch under my one eye ... kept up all day & started again today & sets my anxiety right off :(
Any thoughts? Just praying for the prozac to settle in ❤️

Hi, Jacki! This muscular twitching of the eyelid is a condition called myokymia and is usually caused by "A history of stress, fatigue, and excessive caffeine or alcohol intake". Although muscle twitching of the body in general is a rare side effect of Prozac, I couldn't find anything listed under Ocular side effects.
Have you been stressing more about something or not sleeping well?
Just for the record, I get this too about once or twice a month and it drives me crazy! But it's not a serious disorder.
Yes I have been stressing more ... just so anxious for the Prozac to work ... praying for it daily. You said side effects last about 2 weeks ... and how long to feel some therapeutic working of the Med?? I sure appreciate your insight
Unfortunately, it takes some time for these types of medications to reach optimum therapeutic levels:
"Sleep, energy, or appetite may show some improvement within the first 1-2 weeks. Improvement in these physical symptoms can be an important early signal that the medication is working. Depressed mood, anxiety, and lack of interest in activities may need up to 6-8 weeks to fully improve."
Try to stay positive and try to latch on to those moments that feel good instead of dwelling on the not so good. You're going to have kind of a roller coaster of ups and downs... but the little positives mean that the medication is taking effect. Try not to let your anxiety get the better of you... it's self-defeating. Try doing something that you enjoy every day to take your mind off things, okay?

I seem to feel a bit better later in the day ... but the early am anxiety is always so intense ... Is this common ? Will it pass I pray? Again thank u so much!!
@jacki. Thanks for that! Even though I'm a veterinarian I'm required to read medical journals... and I've also been a patient for most of my life.
As for the anxiety, remember that it can take 6-8 weeks for symptoms, especially anxiety, to fully resolve.
As long as you're seeing improvements it means that the medication is beginning to work.
