Usual Adult Dose for Hypertension
Initial dose: 1 tablet of (chlorthalidone-clonidine 15 mg/0.1 mg) once or twice a day. Determine dosage by individual titration.
Maintenance dose: May increase to a maximum dose of 1 tablet (chlorthalidone-clonidine 15 mg/0.3 mg) twice a day.
Renal Dose Adjustments
Chlorthalidone-clonidine is not recommended in patients with severe renal insufficiency since chlorthalidone is not expected to enter the renal tubule (its site of action) when the glomerular filtration rate is less than 10 mL/min.

Dosing adjustments may be made at weekly intervals if necessary until the desired response is achieved.
Chlorthalidone should be used with caution in severe renal disease. Chlorthalidone should be used with caution in patients with impaired hepatic function.
Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).
Clonidine is not dialyzable by hemo- or peritoneal dialysis.
When discontinuing therapy with clonidine, the dose should be gradually reduced over 2-4 days to avoid withdrawal symptomatology.
