Usual Adult Dose for WHIM Syndrome
Weight over 50 kg: 400 mg orally daily
Weight less than or equal to 50 kg: 300 mg orally daily
Comments:
- Dose should be taken on an empty stomach after an overnight fast, and at least 30 minutes before food.
Use: WHIM Syndrome (warts, hypogammaglobulinemia, infections and myelokathexis)

Renal Dose Adjustments
- Mild and Moderate Renal Dysfunction: No adjustment recommended.
- Severe Renal Dysfunction and End Stage Renal Disease: Not recommended.

- Mild Liver Dysfunction: No adjustment recommended.
- Moderate and Severe Liver Dysfunction: Not recommended.
Dose Adjustments
Use with Strong CYP450 3A4 Inhibitors:
- Reduce daily dosage to 200 mg when used concomitantly.
Precautions
CONTRAINDICATIONS:
- Use with drugs highly dependent on CYP450 2D6 for clearance
Safety and efficacy have not been established in patients younger than 12 years.

Administration advice:
- Take once daily on an empty stomach after an overnight fast, and at least 30 minutes before food.
- Swallow the capsule whole. Do not open, break or chew.
- If a dose is missed, the next dose should be taken as scheduled. Do not take more than one dose each day.
Storage requirements:
- Refrigerate at 2C to 8C (36F to 46F)
- Keep the bottle tightly closed.
- Store in the original container to protect from moisture.
General:
- Advise patients to avoid eating or drinking products with grapefruit.
Monitoring:
- Obtain an EKG to assess QTc at baseline and monitor during treatment as clinically indicated in patients with risk factors.
