Maraviroc (Includes Selzentry) ↔ Hepatotoxicity/Liver Impairment

Applies to: Liver Disease

Hepatotoxicity has been reported with the use of maraviroc. Symptoms of systemic allergic reaction (pruritic rash, eosinophilia, or elevated Ig E), prior to the development of hepatotoxicity may occur. Patients with signs or symptoms of hepatitis or allergic reaction should be evaluated immediately. Additionally, the safety and efficacy of maraviroc has not been studied in patients with significant underlying liver disorders. Caution should be used when administering maraviroc to patients with preexisting liver dysfunction or who are coinfected with viral hepatitis B or C.

Severe Potential Hazard, Moderate plausibility

Applies to: Renal Dysfunction

An increased risk of postural hypotension may occur in patients with severe renal impairment or those with end-stage renal disease, due to increased exposure to maraviroc. This drug should be used with caution in these patients only if they are not receiving a concomitant potent CYP3A inhibitor or inducer, and therapy should be considered only when there is no other treatment option. A dose reduction might be necessary if any symptoms of postural hypotension are experienced. No studies have been performed in patients with severe renal impairment (CrCl < 30 mL/min) or ESRD who are taking concomitant CYP3A inhibitors or inducers, and maraviroc is contraindicated on these patients.

Applies to: Cardiovascular Disease

Maraviroc should be used with caution in patients at increased risk of cardiovascular events as there have been reports of events including myocardial ischemia and infarction in Phase 3 trials.

Moderate Potential Hazard, Moderate plausibility

Applies to: Hypotension, Hypertension

Symptomatic postural hypotension has been frequently reported with maraviroc. Caution should be used when administering maraviroc in patients with a history of postural hypotension or receiving concomitant medication known to lower blood pressure.
