I found the ammonia smell nauseating but my doctor, after denying ammonia is one of the ingredients, had no answer for this. The chemist at Walgreens claimed not to know, either.

Mom just got got a topical analgesic, anti- inflammatory and fever reducer. The last ingredient is “strong ammonia solution.”
Diflofenac sodium (1 %), Carbomer homopolymer Type C, cocoyl caprylocaprate, fragrance, isopropyl alcohol, mineral oil, polyoxyl 20 cetostearyl ether, propylene glycol, purified water, and strong ammonia solution.
I know that ammonia is urea, used in respiratory stimulate - smelling salts, heart imaging - PET scan, gunpowder development, hide and skin tanning, clothing and textile dyeing, jellyfish stings, house cleaning, etc.

Your body heat activates the catalyst in the ammonia and it’s action is in the meat of your flesh, and so the medicine gets warmed up and goes in better.
One of the inactive ingredients is "strong ammonia solution".
It contains the active ingredient, diclofenac sodium, in an opaque, white gel base. Diclofenac sodium is a white to slightly yellow crystalline powder. Diclofenac sodium is a benzeneacetic acid derivative.
The inactive ingredients in Voltaren Gel include: carbomer homopolymer Type C, cocoyl caprylocaprate, fragrance, isopropyl alcohol, mineral oil, polyoxyl 20 cetostearyl ether, propylene glycol, purified water, and strong ammonia solution.
