Voltaren gel is an anti-inflammatory. It's purpose is to reduce inflammation. By reducing inflammation it reduces pain.
I think the answer to this is actually different from person to person & is why they ask those of us in chronic pain to give it a try... giving that the true benefits of this doesn't start right away but with continued & consistent application over a few weeks.
