Tooth loss is not listed as a side effect but the medication can commonly cause dry mouth and tooth decay which I guess might lead to tooth loss, Are the teeth you're losing healthy looking or have you noticed other dental problems?
This doesn't necessarily mean the drug is not the culprit but you might want to make an appointment with a dentist who might be more knowledgeable and who could rule out any other possible causes.

Just another thought, do you smoke? That can cause serious tooth loss.
