Maybe the regulations are different in your state, but in Illinois the max dose of IR is 20mg and max for XR is 30mg. Either way it seems a bit irresponsible of him to put you on two of the 20mg XR's per day, seeing as they are technically a 12hr pill.
-I should say that I am currently taking one 30mg XR in the morning and one 20mg IR around 1:00-2:00 p.m.-
Adderall XR is a time release capsule with two key components. The first is an immediate release component that can be felt anywhere from 5-30 mins after initial ingestion. The second component is the extended portion that doesn't kick in until around 5-6 hours after taking the pill and lasts for another few hours before starting to taper off. I take my 30mg XR at 7:00 a.m. and don't feel it start to lose effect until sometimes 3:00 p.m.
With that being said, if you are taking two of these per day I would expect you to see two major issues:
1. You might be overlapping the second release of the first pill with the initial release of the second pill. In doing this you could expect any of the fun side effects that are always assoiciated with this drug.
2. Your second pill is probably releasing it's time delayed portion right when you're trying to wind down for the night, making it nearly impossible to get any decent sleep.
I'd be curious to know how you've been instructed to take them. I would suggest you talk to him (or better yet your pharmacist) about the possibility of one 20mg XR in the morning and a 20 mg IR as a booster to get you through the rest of the day. This also helps lessen the crash some people have when their XR has run its course. Hope you find a resolution!
