I have been taking this for approximately 5 years. My last visit to my doctor he wrote in my chart to stop taking this medication, but gave no indication as to how to go about this. Cold turkey or just cut back gradually?

Don't even try to do this cold turkey because, as kerry said, you'll likely have discontinuation symptoms which can be severe. Even cutting back on your own can be dangerous if done too quickly or incorrectly.
You need a long withdrawal schedule under the supervision of your doctor. Insist on some instruction from them.
Please get physician help if you are to do this. There can be a withdrawal syndrome associated with SSRIs and SNRIs.
