Using pacritinib together with midostaurin can increase the risk of an irregular heart rhythm that may be serious and potentially life-threatening, although it is a relatively rare side effect. You may be more susceptible if you have a heart condition called congenital long QT syndrome, other cardiac diseases, conduction abnormalities, or electrolyte disturbances (for example, magnesium or potassium loss due to severe or prolonged diarrhea or vomiting). You should seek immediate medical attention if you develop sudden dizziness, lightheadedness, fainting, shortness of breath, or heart palpitations during treatment with these medications, whether together or alone. Midostaurin should be taken with food to help with its absorption. Do not consume grapefruit or grapefruit juice during treatment, as it can significantly increase the blood levels of midostaurin. You may be more likely to experience side effects such as nausea; vomiting; diarrhea; swelling; high blood sugar; heart rhythm abnormalities; and impaired bone marrow function resulting in low numbers of different types of blood cells, which can increase the risk of anemia, bleeding problems, and infections. You should seek medical attention if you develop paleness, fatigue, dizziness, fainting, unusual bruising or bleeding, fever, chills, diarrhea, sore throat, muscle aches, shortness of breath, blood in phlegm, weight loss, red or inflamed skin, body sores, or pain and burning during urination. Talk to your healthcare provider if you have any questions or concerns.

Grapefruit juice may increase the blood levels of pacritinib. This can increase the risk of side effects such as diarrhea, low blood platelet count, infection, and an irregular heart rhythm that may be serious or life-threatening. You should avoid consuming grapefruit and grapefruit juice during treatment with pacritinib. However, if you have been regularly consuming grapefruit or grapefruit juice with the medication, then it is advisable for you to talk with your doctor before changing the amounts of these products in your diet, as this may alter the effects of your medication.
