1 drug interaction and 8 additional warnings found for the following 2 drugs:
- roflumilast
- Zepzelca (lurbinectedin)
Drug interactions
Moderate drug interactions
Moderate:
roflumilast lurbinectedin
Applies to: roflumilast and Zepzelca (lurbinectedin)
Generally avoid
The concomitant use of roflumilast with immunosuppressive agents has not been adequately studied. Roflumilast is a selective phosphodiesterase 4 (PDE4) inhibitor and a nonsteroidal anti-inflammatory agent. In experimental models and in vitro studies, roflumilast has exhibited immunomodulatory effects. Specifically, roflumilast and its active metabolite, roflumilast N-oxide, suppress the release of inflammatory mediators including leukotriene B4, reactive oxygen species, tumor necrosis factor alpha, interferon gamma, and granzyme B. Roflumilast also reduces sputum neutrophils and attenuates influx of neutrophils and eosinophils into the airways.
Management
According to the product labeling for roflumilast in some countries such as Canada and the U.K., concomitant use with immunosuppressive agents such as methotrexate, azathioprine, infliximab, etanercept, and systemic corticosteroids (except when used short-term, e.g., in the treatment of COPD exacerbations) should be avoided if possible. Due to a lack of clinical experience, treatment with roflumilast should not be initiated, or existing treatment should be stopped, in patients receiving immunosuppressive agents and in patients with severe acute infectious diseases, cancers (except basal cell carcinoma), or severe immunological diseases (e.g., HIV infection, multiple sclerosis, lupus erythematosus, progressive multifocal leukoencephalopathy).

Food/lifestyle interactions
Major food/lifestyle interactions
Major:
lurbinectedin Food/Lifestyle
Applies to: Zepzelca (lurbinectedin)
Generally avoid
Grapefruit and Seville oranges may increase the plasma concentrations of lurbinectedin. The proposed mechanism is inhibition of CYP450 3A4-mediated first-pass metabolism in the gut wall by certain compounds present in grapefruit and Seville oranges. Inhibition of hepatic CYP450 3A4 may also contribute. The interaction has not been studied with grapefruit or Seville oranges, but pharmacokinetic data are available for potent and moderate CYP450 3A4 inhibitors. In a clinical drug interaction study, coadministration of itraconazole, a potent CYP450 3A4 inhibitor, increased the systemic exposure (AUC) of total lurbinectedin by 2.7-fold and unbound lurbinectedin by 2.4-fold. In a Phase 1 study, coadministration of aprepitant, a moderate CYP450 3A4 inhibitor, decreased lurbinectedin plasma clearance by 33% compared to lurbinectedin alone. In general, the effect of grapefruit and Seville oranges is concentration-, dose- and preparation-dependent, and can vary widely among brands. Certain preparations of grapefruit juice and Seville oranges (e.g., high dose, double strength) have sometimes demonstrated potent inhibition of CYP450 3A4, while other preparations (e.g., low dose, single strength) have typically demonstrated moderate inhibition. Increased exposure may increase the incidence and severity of adverse reactions of lurbinectedin, such as myelosuppression and hepatotoxicity.
Management
Patients should avoid consumption of grapefruit, grapefruit juice, and Seville oranges during treatment with lurbinectedin.

Minor food/lifestyle interactions
Minor:
roflumilast Food/Lifestyle
Applies to: roflumilast
Food intake does not affect the total exposure to roflumilast and its pharmacologically active N-oxide metabolite, but delays the time to maximum concentration (Tmax) of roflumilast by one hour and reduces its peak plasma concentration (Cmax) by approximately 40%. The Tmax and Cmax of
roflumilast N-oxide are unaffected. Roflumilast may be taken with or without food.

Disease interactions
6 potential interactions Applies to: 5 conditions, including Depression, Anxiety and Stress, Liver Disease, Failure to Thrive, and Bone Marrow Depression/Low Blood Counts.
Anxiety and Stress
Moderate:
roflumilast
Roflumilast has a moderate potential interaction with Anxiety and Stress.
The use of roflumilast is associated with an increase in psychiatric adverse reactions. Instances of suicidal ideation and behavior, including completed suicide, have been observed in clinical trials. Before using roflumilast in patients with a history of depression and/or suicidal thoughts or behavior, prescribers should carefully weigh the risks and benefits of treatment in all patients. Close monitoring for the emergence or worsening of insomnia, anxiety, depression, suicidal thoughts or other mood changes is recommended.
Bone Marrow Depression/Low Blood Counts
Moderate:
Zepzelca
The lurbinectedin in Zepzelca has a moderate potential interaction with Bone Marrow Depression/Low Blood Counts.
The use of lurbinectedin can cause myelosuppression. Assess blood counts, including neutrophil count and platelet count before each therapy and administer lurbinectedin only to patients with baseline neutrophil count of at least 1,500 cells/mm3 and platelet count of at least 100,000/mm3. For neutrophil count less than 500 cells/mm3 or any value less than lower limit of normal, the use of G-CSF is recommended. Withhold, reduce the dose, or permanently discontinue lurbinectedin based on severity.
Depression
Moderate:
roflumilast
Roflumilast has a moderate potential interaction with Depression.
Failure to Thrive
Moderate:
roflumilast
Roflumilast has a moderate potential interaction with Failure to Thrive.
Weight loss is a common adverse reaction with the use of roflumilast. Patients treated with roflumilast should have their weight monitored regularly. If unexplained or clinically significant weight loss occurs, weight loss should be evaluated, and discontinuation of treatment should be considered. Care should be exercised when using this agent in patients at risk of weight loss.
Liver Disease
Major:
roflumilast
Roflumilast has a major potential interaction with Liver Disease.
Roflumilast is contraindicated in patients with moderate to severe liver impairment (Child-Pugh B or C). Roflumilast is extensively metabolized via Phase I (cytochrome P450) and Phase II (conjugation) reactions. Clinicians should consider the risk-benefit of administering roflumilast to patients with mild liver impairment (Child-Pugh A).
Moderate:
Zepzelca
The lurbinectedin in Zepzelca has a moderate potential interaction with Liver Disease.
The use of lurbinectedin can cause hepatotoxicity. Assess liver function tests, before administering lurbinectedin, periodically during treatment, and as clinically indicated. No dose adjustment is recommended for patients with mild hepatic impairment. Caution is recommended when using this agent in patients with moderate or severe hepatic impairment as the pharmacokinetics of lurbinectedin have not been studied in these patients. Withhold, reduce the dose, or permanently discontinue lurbinectedin based on severity.
