Interactions between Larotrectinib and Sorafenib
1 drug interaction and 6 additional warnings found for the following 2 drugs:
- larotrectinib
- sorafenib
Drug interactions
Moderate drug interactions
Moderate:
SORAfenib larotrectinib
Applies to: sorafenib and larotrectinib
Monitor
Based on in vitro inhibition data, coadministration with sorafenib may increase the plasma concentrations of drugs that are substrates of the P-glycoprotein (P-gp) transporter. The proposed mechanism is decreased clearance in the intestine, liver, and/or kidney due to inhibition of P-glycoprotein-mediated drug efflux by sorafenib.
Management
Caution is advised when sorafenib is used concurrently with drugs that are known P-gp substrates, particularly those with a narrow therapeutic range. Dosage adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever sorafenib is added to or withdrawn from therapy.

Therapeutic duplication warnings
Therapeutic duplication is the use of more than one medicine from the same drug category or therapeutic class to treat the same condition.
It can be intentional, when combining drugs with similar actions adds demonstrated therapeutic benefit — or unintentional, such as when a patient sees multiple prescribers or fills prescriptions at more than one pharmacy, which can lead to adverse effects.
Duplication:
Multikinase inhibitors
The recommended maximum number of medicines in the 'multikinase inhibitors' category to be taken concurrently is usually one. Your list includes two medicines belonging to the 'multikinase inhibitors' category:
- larotrectinib
- sorafenib
Note: In certain circumstances, the benefits of taking this combination of drugs may outweigh any risks. Always consult your healthcare provider before making changes to your medications or dosage.
Food/lifestyle interactions
Moderate food/lifestyle interactions
Moderate:
SORAfenib Food/Lifestyle
Applies to: sorafenib
Adjust dosing interval
Food may reduce the oral absorption and bioavailability of sorafenib. According to the product labeling, sorafenib bioavailability was reduced by 29% when administered with a high-fat meal compared to administration in the fasted state. When given with a moderate-fat meal, bioavailability was similar to that in the fasted state.
Management
To ensure maximal and consistent oral absorption, sorafenib should be taken at least one hour before or two hours after eating.

Generally avoid
Grapefruit juice may increase the plasma concentrations of larotrectinib. The proposed mechanism is inhibition of CYP450 3A4-mediated metabolism of larotrectinib by certain compounds present in grapefruit. When a single 100 mg dose of larotrectinib was coadministered with itraconazole, a potent CYP450 3A4 inhibitor, larotrectinib peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 2.8- and 4.3-fold, respectively, compared to administration of larotrectinib alone. The interaction has not been studied with grapefruit juice. In general, the effect of grapefruit juice is concentration-, dose- and preparation-dependent, and can vary widely among brands. Certain preparations of grapefruit juice (e.g., high dose, double strength) have sometimes demonstrated potent inhibition of CYP450 3A4, while other preparations (e.g., low dose, single strength) have typically demonstrated moderate inhibition. Increased exposure to larotrectinib may increase the risk of adverse effects such as neurotoxicity (delirium, dysarthria, dizziness, gait disturbance, paraesthesia, encephalopathy, memory impairment, tremor) and hepatotoxicity (elevations in liver transaminases). Food does not alter the pharmacokinetics of larotrectinib to a clinically significant extent. When a single 100 mg dose of larotrectinib was administered with a high-fat meal (approximately 900 calories; 58 g carbohydrate, 56 g fat, 43 g protein) in healthy study subjects, larotrectinib peak plasma concentration (Cmax) was reduced by 35% while systemic exposure (AUC) was similar compared to administration in the fasted state.
Management
Larotrectinib may be taken with or without food. Patients should avoid the consumption of grapefruit and grapefruit juice during treatment.

Disease interactions
3 potential interactions Applies to: 3 conditions, including Liver Disease, Interstitial Lung Disease, and Pulmonary Impairment.
Interstitial Lung Disease
Moderate:
sorafenib
SORAfenib has a moderate potential interaction with Interstitial Lung Disease.
The use of certain multikinase inhibitors has been associated with pulmonary toxicity. Serious cases of interstitial lung disease (ILD), including fatal cases and interstitial pneumonitis or pulmonary fibrosis have been reported. Caution is recommended when using these agents in patients with a history of interstitial pneumonitis or pulmonary fibrosis or those patients presenting with acute onset of new or progressive unexplained pulmonary symptoms such as dyspnea, cough, and fever pending diagnostic evaluation. If ILD is confirmed, these agents should be permanently discontinued and appropriate measures should be instituted. Treatment should be immediately withheld in patients diagnosed with ILD/pneumonitis and permanently discontinued if no other potential causes of ILD/pneumonitis have been identified.
Liver Disease
Moderate:
larotrectinib
Larotrectinib has a moderate potential interaction with Liver Disease.
Forty- five percent of the patients receiving larotrectinib had increased hepatic parameters, including Grade 3 increased AST or ALT in 6% of patients. Monitor liver tests, including ALT and AST, every 2 weeks during the first month of treatment, then monthly thereafter, and as clinically indicated. Treatment should be withheld or permanently discontinued based on the severity of hepatic toxicity. Caution is advised in patients with moderate to severe hepatic impairment as the drug clearance was reduced in these patients (Child-Pugh B and C). Dose should be reduced on these patients as recommended by the manufacturers (refer to prescribing information).
Pulmonary Impairment
Moderate:
sorafenib
SORAfenib has a moderate potential interaction with Pulmonary Impairment.
