Pretomanid Disease Interactions

There are 2 disease interactions with pretomanid.

Moderate

Pretomanid (applies to pretomanid) hepatic dysfunction

Moderate Potential Hazard, Moderate plausibility. Applicable conditions: Liver Disease, Alcoholism

The use of pretomanid with the combination regimen of bedaquiline and linezolid has been associated with hepatic adverse reactions. It is recommended to monitor for signs and symptoms of hepatotoxicity and assess liver test at baseline, at two weeks, and then monthly while on treatment and as needed. Interrupt therapy as recommended by the manufacturer and avoid alcohol and hepatotoxic agents, especially in patients with hepatic impairment.

Moderate

Pretomanid (applies to pretomanid) QT prolongation

Moderate Potential Hazard, Moderate plausibility. Applicable conditions: Arrhythmias, Electrolyte Abnormalities, Hypothyroidism, Hypocalcemia, Hypokalemia, Magnesium Imbalance, Congestive Heart Failure

The use of pretomanid with the combination regimen of bedaquiline and linezolid has been associated with QT prolongation. It is recommended to obtain an ECG before starting treatment and at least 2, 12, and 24 weeks during treatment. Obtain serum potassium, calcium, and magnesium at baseline and correct if abnormal. Monitor these electrolytes if QT prolongation is detected. Exercise care when using this agent in patients with a history of Torsade de Pointes, congenital long QT syndrome, hypothyroidism, bradyarrhythmia, uncompensated heart failure, or serum calcium, magnesium, or potassium levels below the lower limits of normal. If syncope occurs, obtain an ECG to detect QT prolongation. Discontinue treatment if clinically significant ventricular arrhythmia or a corrected QT interval by Fredericia (QTcF) interval of greater than 500 milliseconds, as confirmed by repeat ECG, develops.

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
