The pharmacokinetic disposition of fomepizole has not been determined in patients with hepatic and/or renal impairment. Fomepizole is extensively metabolized by the liver. Following multiple dosing, fomepizole induces its own metabolism and the rate of elimination is increased. About 1% to 3.5% of fomepizole is eliminated unchanged in the urine. Therapy with fomepizole should be administered cautiously in patients with compromised hepatic and/or renal function. Fomepizole is dialyzable and the frequency of dosing should be increased to every 4 hours during hemodialysis.
