... constant state of genital fullness and arousal. I never get an erection suitable for penetration. However, from time to time I will wake up with a firm and hard erection. It soon deflates but this is evidence that the ability is still there. The question is how do I achieve that kind of erection when I need it?

I believe your dosage is too low! It is totally in the manufacturer's guidelines to up your dose to 10 mg/daily, or even the 20 mg/daily maximum recommended dosage!
I when you are in full arousal during night (3am-9am-testosterone is at highest daily point), that is the time to take Cialis again. Sometimes I take just a low dose sublingually. Be a little patient, and you can achieve full penetration and orgasm. You might add L-arginine and L-citrulline too to night meds.
I'm going to answer, Tex, as best as I can, but you may not like what you hear. And let's be clear. I mean you no disrespect whatsoever. I also want to say a few things to my "brothers-in-suffering." Look at the ratings for this drug, or ANY drug that purportedly treats BPH. There is a correlation between the ratings and the age brackets. In other words, the younger the guy, the greater the likelihood that he'll rate this drug highly. Positive ratings from older guys? Not so much. The reality is such that, because of our age, we're behind the medical curve in getting relief. I've read a lot of comments, not only for Cialis, but for other drugs that supposedly treat BPH. You name it. I've read it. Tender breasts, shrinking penis, decreased ejaculate, ED, etc. I do believe the veracity of every comment I've read.
Speaking of reading, I might suggest picking up a used copy of the "Prostate Book," by Peter Scardino, MD, or "The Whole Life Prostate Book," by Carter Ballentine, MD. Both are excellent, and they're available on Amazon. The takeaway, especially from Dr. Ballentine's book is simply this: Don't underestimate the power in taking care of oneself. We're older, gentlemen. Few of us are running marathons these days. That doesn't mean we throw in the towel. Be honest. Are you exercising at all? As for dietary advice, Michael Pollan says it best in seven succinct words: "Eat FOOD. Not too much. Mostly plants." Are you sleeping adequately? As for supplements, saw palmetto is all the rage. Didn't work for me. That's not to say that it won't work for you, despite the fact that science has deemed saw palmetto "no more effective than placebo." What DOES work for me, and what IS supported by the literature is pygeum, taken in combination with beta sitosterol (forget the one that's heavily advertised on TV). Finally, take heart. There are VERY promising therapies on the horizon. Botox, reducing angiogenesis to the prostate, and new drugs in the pipeline. Finally - and don't take offense - lose weight. It truly helps. Health care - is self care. Not a health care professional. Start exercising, Tex. Doesn't have to be strenuous, but it should be aerobic, and it should be consistent. Hope this helps.
