yes it is. Most drugs have this salt name after them, but are typically dropped for convenience sake unless there is a difference in the salt name.

No, you don't need to to request another one prescription, metformin always comes in the form of hydrochloric salt (hydrochloride), sometimes the doctors writes only metformin, however metformin always comes as a hydrochloric salt
