by 'sick' I mean heart attack like symptoms: nausea, vomiting, sweating, heart palpitations and inability to draw a breath. I can't take even 5mg of hydro without feeling like I am about to have a heart attack. But when I am in severe enough pain (gastrointestinal), I can take an oxycodone 5/375 acetaminophen and the pain subsides without making me feel the heart attack like side effects.
Why is this?

It doesn't sound much like an allergy, just side effects. There is no explanation of why a person tolerates one med and not another.
I have to agree with Kaismama, what you describe are not allergy effects but rather an intolerance. Hydrocodone and oxycodone are two different drugs even though they are both opioids and who knows why one is tolerated better than another? Just stay away from the hydrocodone because you dont tolerate the side effects well.
A lot of people have this reaction to one drug and not another due to the the need to have other chemicals put in with these medications. These are referred to as binders and everyone reacts differently to them.
The brand name and generic products usually have the same main ingredient, but differ in their quality of binders. Also each person has different liver enzyme function, which controls how the drug is metabolized.
That's a bit extreme difference. I know many folks do not react well to Hyrdocodone and Oxycodone is stronger, however, you may just be allergic to the Hydrocodone, my concern is the symptoms you report are a but unusual and should be checked out to rule out the possibility of it being more than an allergic reaction.
