I have been using OTC diphenhydramine nightly for about 6 months. It works well but have begun to have lower feet swelling.
Would diphenhydramine 50mg nightly be causing bilateral lower extremity swelling?

Hello,
Peripheral edema is not listed as a possible side effect.
I suggest you see/talk to a medical professional, there could potentially be an underlying condition as possible side effects are commonly experienced by most people at the start of their treatment. However as everyone is diffedrent medications and everything that comes with them (benefits, risks, potential interactions, possible side effects, etc.) could affect some patients differently, but I would definitely see a medical professional.
