If you are taking nitrofurantoin for UTI, your bacteria is very likely to be ESBL E. Coli. Carbapenems are more effective in treating this condition. Although some doctors state that Amoxicillin 875mg is a very good antibiotic to treat UTI, there are more than 200 types of ESBL enzymes which is hard to tell if Amoxicillin 875mg is a good choice. If you have cultures done, consult with your doctor as to what type of ESBL enzymes E. Coli you are having, then he/she can explore other options for you.
I’m afraid this is wrong in two ways.
Firstly a prescription for nitrofurantoin does not mean the patient has ESBL. Nitrofurantoin is now the first line treatment for uncomplicated UTI in adults for both ESBL and other UTI causing organisms.
Secondly amoxicillin will not treat ESBL. The definition of ESBL is that it is resistant to penicillins. Amoxicillin is a penicillin.
The OP would indeed be best off consulting with their doctor and having urine cultures done if nitrofurantoin is ineffective.
