I’m on 40mg of fluoxetine daily (supposed to be 60mg but managing on 40) for anxiety and depression but struggle quite regularly with muscle spasms during the night so doctor has put me on 10mg daily of amitriptyline at night time to help stop the muscle spasms but as I said I struggle with anxiety and I’m worried about mixing both medications, has anyone else had this combination that could offer some advice please?

Fluoxetine is a moderately strong inhibitor of the CYP450 enzymes 1A2 and 2C19 which are involved in the metabolism of Amitriptyline, and a very strong inhibitor of 2D6 which is necessary for the final stage of metabolizing Amitriptyline and its active metabolite Nortriptyline. So while combining Amitriptyline with Fluoxetine could lead to increased plasma levels of Amitriptyline (and Nortriptyline), research suggests that this might not occur when lower doses of these medications are used, likely because the extent of enzyme inhibition by SSRIs is dose dependent. 10mg of Amitriptyline is a very low dose, and 40mg of Fluoxetine is in the middle of the therapeutic range. You can get tested to find out if genetically you are a slow metabolizer of Amitriptyline, ideally before you start taking it, because levels could be much higher than expected... but at such a low dose it probably wouldn't matter. I take 75mg Amitriptyline to help with sleep. (Gelenberg, 2010; Vezmar, 2009; Palaniyappan, 2009). -Scott
Its such a small dose of amitriptyline that it shouldn't cause any issues. The main concern would be serotonin syndrome, but you are on such a low dose of amitriptyline so they shouldnt intervene with the fluoxetine.

By the way. I take amitriptyline for muscle spasms too. I was on both of these together but recently changed from fluoxetine to citalopram. I recommend taking amitriptyline just before bed or 3 hours before sleep if you have sleep issues
And you didn’t have no side affects/ reactions?
None at all, but if I take the amitriptyline too early it makes me really tired. I take it 2-3 hours before bed. No other symptoms. 10mg is a really low dose. Dont worry :)
