Moderate drug interactions

Moderate:

roflumilast belimumab

Applies to: roflumilast and belimumab
Generally avoid

The concomitant use of roflumilast with immunosuppressive agents has not been adequately studied. Roflumilast is a selective phosphodiesterase 4 (PDE4) inhibitor and a nonsteroidal anti-inflammatory agent. In experimental models and in vitro studies, roflumilast has exhibited immunomodulatory effects. Specifically, roflumilast and its active metabolite, roflumilast N-oxide, suppress the release of inflammatory mediators including leukotriene B4, reactive oxygen species, tumor necrosis factor alpha, interferon gamma, and granzyme B. Roflumilast also reduces sputum neutrophils and attenuates influx of neutrophils and eosinophils into the airways.

Management

According to the product labeling for roflumilast in some countries such as Canada and the U.K., concomitant use with immunosuppressive agents such as methotrexate, azathioprine, infliximab, etanercept, and systemic corticosteroids (except when used short-term, e.g., in the treatment of COPD exacerbations) should be avoided if possible. Due to a lack of clinical experience, treatment with roflumilast should not be initiated, or existing treatment should be stopped, in patients receiving immunosuppressive agents and in patients with severe acute infectious diseases, cancers (except basal cell carcinoma), or severe immunological diseases (e.g., HIV infection, multiple sclerosis, lupus erythematosus, progressive multifocal leukoencephalopathy).

Food/lifestyle interactions

Minor food/lifestyle interactions

Minor:

roflumilast Food/Lifestyle

Applies to: roflumilast

Food intake does not affect the total exposure to roflumilast and its pharmacologically active N-oxide metabolite, but delays the time to maximum concentration (Tmax) of roflumilast by one hour and reduces its peak plasma concentration (Cmax) by approximately 40%. The Tmax and Cmax of
roflumilast N-oxide are unaffected. Roflumilast may be taken with or without food.

Disease interactions

8 potential interactions Applies to: 7 conditions, including Depression, Anxiety and Stress, Liver Disease, Infection, and Immunodeficiency.

Anxiety and Stress

Moderate:

roflumilast

Roflumilast has a moderate potential interaction with Anxiety and Stress.

The use of roflumilast is associated with an increase in psychiatric adverse reactions. Instances of suicidal ideation and behavior, including completed suicide, have been observed in clinical trials. Before using roflumilast in patients with a history of depression and/or suicidal thoughts or behavior, prescribers should carefully weigh the risks and benefits of treatment in all patients. Close monitoring for the emergence or worsening of insomnia, anxiety, depression, suicidal thoughts or other mood changes is recommended.

Depression

Moderate:

belimumab

Belimumab has a moderate potential interaction with Depression.

In clinical trials, depression and suicidality were reported in patients receiving belimumab. Before treatment with belimumab, the risk of depression and suicide should be assessed, considering the patient's medical history and current psychiatric status; patients should continue to be monitored during therapy. Patients receiving belimumab (and caregivers, if applicable) should be instructed to contact their health care provider if they experience new or worsening depression, suicidal thoughts or behaviors, or other mood changes. The risk and benefit of continued therapy should be considered for patients who develop such symptoms.

Moderate:

roflumilast

Roflumilast has a moderate potential interaction with Depression.

Failure to Thrive

Moderate:

roflumilast

Roflumilast has a moderate potential interaction with Failure to Thrive.

Weight loss is a common adverse reaction with the use of roflumilast. Patients treated with roflumilast should have their weight monitored regularly. If unexplained or clinically significant weight loss occurs, weight loss should be evaluated, and discontinuation of treatment should be considered. Care should be exercised when using this agent in patients at risk of weight loss.

Immunodeficiency

Moderate:

belimumab

Belimumab has a moderate potential interaction with Immunodeficiency.

Cases of John Cunningham virus (JC virus)-associated progressive multifocal leukoencephalopathy (PML) resulting in neurological deficits (including fatal cases) have been reported in patients with systemic lupus erythematosus receiving immunosuppressants (including belimumab). Risk factors for PML include treatment with immunosuppressant therapies and impairment of immune function. The diagnosis of PML should be considered in any patient presenting with new-onset or deteriorating neurological signs/symptoms; a neurologist or other appropriate specialist should be consulted as clinically indicated. If PML is confirmed, belimumab should be discontinued.

Infection

Moderate:

belimumab

Belimumab has a moderate potential interaction with Infection.

Serious and sometimes fatal infections have been reported in patients receiving immunosuppressive agents (including belimumab). Overall, while the incidence of serious infections in controlled trials was similar between belimumab and placebo, fatal infection occurred more often with belimumab. The risk and benefit should be considered before starting treatment with belimumab in patients with severe or chronic infections. Interruption of therapy should be considered in patients who develop a new infection while receiving belimumab and these patients should be monitored closely.

Liver Disease

Major:

roflumilast

Roflumilast has a major potential interaction with Liver Disease.

Roflumilast is contraindicated in patients with moderate to severe liver impairment (Child-Pugh B or C). Roflumilast is extensively metabolized via Phase I (cytochrome P450) and Phase II (conjugation) reactions. Clinicians should consider the risk-benefit of administering roflumilast to patients with mild liver impairment (Child-Pugh A).

Psychiatric Disorders

Moderate:

belimumab

Belimumab has a moderate potential interaction with Psychiatric Disorders.

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
