Cefmetazole Dosage
Applies to the following strengths: 1 g; 2 g; 1 g/50 mL; 2 g/50 mL

Usual Adult Dose for Hysterectomy
Abdominal approach:
1 g IV 30 to 90 minutes before surgery and repeat 8 and 16 hours later.
Vaginal approach:
2 g IV as a single dose 30 to 90 minutes before surgery or 1 g IV 30 to 90 minutes before surgery and repeat 8 and 16 hours later.
Usual Adult Dose for Cesarean Section
2 g IV as a single dose after clamping cord or 1 g IV after clamping cord and repeat 8 and 16 hours later.
Usual Adult Dose for Surgical Prophylaxis
Colorectal surgery:
2 g IV 30 to 90 minutes before surgery and may be repeated 8 and 16 hours later.
Cholecystectomy (high risk):
1 g IV 30 to 90 minutes before surgery and repeat 8 and 16 hours later.
Renal Dose Adjustments
CrCl<10 mL/min: 1 to 2 g IV every 48 hours.
CrCl 10 to 29 mL/min: 1 to 2 g IV every 24 hours.
CrCl 30 to 49 mL/min: 1 to 2 g IV every 16 hours.
CrCl 50 to 90 mL/min: 1 to 2 g IV every 12 hours.
Liver Dose Adjustments
No adjustment recommended
Dialysis
Administer dose after hemodialysis.
