I am 33 weeks pregnant with severe heartburn, my OB prescribed 20mg of Protonix because its the only thing that used to work. Its a lower dose than what I used to take. I'm on day 4, how long should I wait until asking the doc to give me a higher dose? It is so unbearable that I can only eat applesauce with no problem :(

You can go ahead and let your OB know that it still isnt providing relief. It may just be the baby crowding the stomach that is the problem. Have you felt any relief at all? Have you tried eating slowly small amounts of food several times a day? You may ask him(or her) if you can drink something like Boost or Ensure so that you get some nutrition on board for your little one as this may be a problem only relieved by delivery.
If I eat nothing but applesauce, it helps somewhat. I ate applesauce and a yogurt for breakfast, a few slices of pears an hour later, and then another yogurt after another hour. Then had heartburn :( I don't know what else to do but eat a little applesauce every 3 hrs or so :(
Ask him if it is ok for you to drink something like Boost or Ensure. These are drinks that have all the nutrition you need but being liquid they may not cause you heartburn as bad because they wont crowd your stomach so much. This way you and the baby are getting the nutrients you need. Are you able to take your prenatal vitamin? Applesauce and yogurt arent going to give you the protein you need. Try eating soft foods. Can you eat cottage cheese? Peanut butter? Try to eat small amounts but frequently. Perhaps some TUMS after eating will help more immediately with the acid. TUMS are just calcium so they wont hurt your baby.
Also, be sure that you are sitting up for at least an hour after eating. If you lay down, your stomach contents are going to crawl right up your throat so be sure that you keep your head elevated after you eat so either sit up or prop yourself up with pillows in bed. Some women find it helps them to raise the top of the bed by propping the upper legs up on books or bricks but if that isnt feasible keep your head and torso propped up with pillows behind you so that you keep your top half elevated or lay down with a wedge cushion behind you. Drink plenty of fluids to keep the stomach acid diluted-preferably water. Water will help the food move through you faster as well-it kind of washes it through.
Tums dont help and anything with a lot of fat doesn't either. Looks like the next few weeks are going to be hard and painful :(
It sounds to me like your baby is really crowding your stomach. Once he drops a bit a few weeks before delivery you will feel better. Hang in there. It is not too much longer and just try to nibble on some foods with protein so you can heal well after delivery. Hopefully he will drop down and engage for delivery soon and give you some relief. Pregnancy can be miserable sometimes.
I don't know what's up, because my daughter never crowded my stomach when I was pregnant with her, its becoming unmanageable and I'm calling my OB today to let her know. :(
