Is it safe for me to use Azithromycin 250 with my condition and I’m also on Coumadin 4 mg.I have a bad cold so the doctor prescribed this to me. I’m afraid to take it because it’s a new medication for me since I had open heart surgery 2 years ago.

Azithromycin interacts with 4 diseases, Hypertrophic cardiomyopathy is not one of them but I am not a medical professional and there is one potential moderate interaction with QT Prolongation, please click on the link below to read which ones are:
Moderate meaning:
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Moderate
Azithromycin (applies to azithromycin) QT prolongation
Moderate Potential Hazard, Moderate plausibility. Applicable conditions: Magnesium Imbalance, Arrhythmias, Congestive Heart Failure, Hypokalemia
Prolonged cardiac repolarization and QT interval have been reported in patients receiving treatment with azithromycin. Providers should consider the risk of QT prolongation, which can be fatal, when weighing the risks and benefits of azithromycin for at-risk patients including: patients with known prolongation of the QT interval, history of torsades de pointes, congenital long QT syndrome, bradyarrhythmias, or uncompensated heart failure; patients with ongoing proarrhythmic conditions such as uncorrected hypokalemia or hypomagnesemia, clinically significant bradycardia, or receiving other drugs that prolong the QT interval.

If in doubt, I do recommend that before you take Azithromycin you contact the medical professional who prescribed the antibiotic.
Interactions between your drugs
Moderate
warfarin azithromycin
Applies to: Coumadin (warfarin), azithromycin
Using azithromycin together with warfarin may cause you to bleed more easily. You may need a dose adjustment based on your prothrombin time or International Normalized Ratio (INR). Call your doctor promptly if you have any unusual bleeding or bruising, vomiting, blood in your urine or stools, headache, dizziness, or weakness. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Most likely the Dr. who prescribed the antibiotic is aware of the potential interaction between both medications and has outweighed that the benefit is greater than the potential risk and would not put you in harms way.
Again, if in doubt do not hesitate to contact your Dr. before you decide to take the antibiotic.
