We know it has to be diluted because it's very potent and will numb your mouth and tongue sometimes even for days.
I don't know for sure why they suggest a flavored diluent but I'm guessing it probably tastes pretty nasty and the juice or whatever would make it more palatable.
You could try asking your pharmacist... they would probably know...

P.S. I just read that it's peppermint flavored? That blows my theory...
I just tried to research this question too and found no explanation either.
Maybe someone using the concentrate will chime in if they know why.
