Drug interactions
Moderate drug interactions
Moderate:
filgrastim ibrutinib
Applies to: Neupogen (filgrastim) and ibrutinib
Adjust dosing interval
The safety and efficacy of hematopoietic growth factors such as colony-stimulating factors (G-CSF and GM-CSF) and stem cell factors (SCF) given simultaneously with cancer chemotherapy have not been established. Theoretical concerns exist regarding their concomitant administration because hematopoietic growth factors stimulate myeloid cell proliferation while antineoplastic agents primarily target rapidly dividing cells.
Management
Because of the potential sensitivity of rapidly dividing myeloid cells to cancer chemotherapy, hematopoietic growth factors should not be used within 24 hours before or 24 hours after administration of antineoplastic agents.

Food/lifestyle interactions
Major food/lifestyle interactions
Major:
ibrutinib Food/Lifestyle
Applies to: ibrutinib
Generally avoid
Coadministration with grapefruit, grapefruit juice, or Seville oranges may significantly increase the plasma concentrations of ibrutinib. The proposed mechanism is inhibition of CYP450 3A4-mediated first-pass metabolism in the gut wall by certain compounds present in grapefruit. Pharmacokinetic modeling suggests that other moderate CYP450 3A4 inhibitors such as diltiazem and erythromycin may increase ibrutinib systemic exposure (AUC) by 6- to 9-fold under fasting condition. The safety and efficacy of these exposures are unknown. The highest ibrutinib dose evaluated in clinical trials was 12.5 mg/kg (actual doses of 840 to 1400 mg) given for 28 days, which yielded single dose AUC values that were approximately 50% greater than steady-state exposures seen at the highest indicated dose of 560 mg.
Adjust dosing interval
Food increases the oral bioavailability of ibrutinib. The mechanism of interaction is unknown. According to the product labeling, administration with food increases ibrutinib exposure approximately 2-fold compared to administration after overnight fasting.
Management
Patients treated with ibrutinib should avoid consumption of Seville oranges, grapefruit, grapefruit juice, and any supplement containing grapefruit extract. Ibrutinib should be taken once daily at approximately the same time each day.

Bleeding Associated with Coagulation Defect
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with Bleeding Associated with Coagulation Defect.
Cases of bleeding (including bruising, petechia, intracranial hemorrhage [including subdural hematoma], gastrointestinal bleeding, hematuria, and postprocedural hemorrhage), including fatal bleeding events, have been reported in patients treated with ibrutinib. Risk of major hemorrhage is increased in patients receiving antiplatelet or anticoagulant therapy; these patients should be monitored for signs/symptoms of bleeding. It is recommended to consider the benefit-risk of withholding ibrutinib for at least 3 to 7 days pre- and post-surgery depending upon the type of surgery and the risk of bleeding.

Fatal and serious cardiac arrhythmias and cardiac failure have occurred with ibrutinib. These adverse reactions occurred in patients with and without preexisting hypertension or cardiac comorbidities; patients with cardiac comorbidities may be at greater risk of these events. Ventricular tachyarrhythmias, atrial fibrillation, and atrial flutter have occurred in patients treated with ibrutinib, especially in patients with acute infections or cardiac risk factors (including hypertension, diabetes mellitus, history of cardiac arrhythmia). Cardiac history and function should be evaluated at baseline, and patients should be monitored for cardiac arrhythmias and cardiac function. Further evaluation (e.g., ECG, echocardiogram) should be obtained as indicated for patients who develop symptoms of arrhythmia (e.g., palpitations, lightheadedness, syncope, chest pain), new onset dyspnea, or other cardiovascular concerns. Cardiac arrhythmias and cardiac failure should be managed appropriately, dosage modification guidelines should be followed, and the risks and benefits of continued ibrutinib treatment should be considered.

History, Thrombotic/Thromboembolic Disorder
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with History, Thrombotic/Thromboembolic Disorder.
Cases of bleeding (including bruising, petechia, intracranial hemorrhage [including subdural hematoma], gastrointestinal bleeding, hematuria, and postprocedural hemorrhage), including fatal bleeding events, have been reported in patients treated with ibrutinib. Infection
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with Infection.
Fatal and nonfatal infections (including bacterial, viral, or fungal) have occurred with ibrutinib therapy. Cases of progressive multifocal leukoencephalopathy (PML) and Pneumocystis jirovecii pneumonia (PJP) have been reported in patients treated with ibrutinib. Prophylaxis (according to standard of care) should be considered in patients at increased risk for opportunistic infections. Patients should be monitored and evaluated for fever and infections and treated appropriately.
Leukocytosis
Major:
Neupogen
The filgrastim in Neupogen has a major potential interaction with Leukocytosis.
Filgrastim and pegfilgrastim are growth factors for neutrophil progenitor cells. Therefore, these should not be used in patients with leukocytosis. If excessive white blood cell counts occur during CSF therapy, treatment should be interrupted or the dosage reduced.
Liver Disease
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with Liver Disease.
Ibrutinib is metabolized in the liver. In patients with B-cell malignancies, ibrutinib should be avoided in those with severe liver dysfunction (Child-Pugh C). The recommended dosage should be reduced when administering ibrutinib to patients with mild or moderate liver dysfunction (Child-Pugh A and B); more frequent monitoring for ibrutinib adverse reactions is recommended for these patients.
In patients with chronic graft-versus-host disease, ibrutinib should be avoided in those with total bilirubin level greater than 3 times the upper limit of normal (3 x ULN), unless of nonhepatic origin or due to Gilbert's syndrome. The recommended dosage should be reduced when administering ibrutinib to patients with total bilirubin level greater than 1.5 to 3 x ULN, unless of nonhepatic origin or due to Gilbert's syndrome.
Myeloproliferative Disorders
Major:
Neupogen
The filgrastim in Neupogen has a major potential interaction with Myeloproliferative Disorders.
Colony stimulating factors primarily stimulates the proliferation of neutrophils but may also, theoretically, enhance tumor growth, particularly in myeloid malignancies. Therapy with these drugs should be administered cautiously in patients with myeloid tumors. Additionally, if used for PBPC (peripheral blood progenitor cell) collection in such patients, tumor cells may be released from the marrow and collected in the leukapheresis product. The effect of reinfusion of tumor cells is uncertain at this time.
Renal Dysfunction
Major:
Neupogen
The filgrastim in Neupogen has a major potential interaction with Renal Dysfunction.
Although it has been reported that renal dysfunction has no effect on the pharmacokinetics of colony stimulating factors (CSF), filgrastim and pegfilgrastim may cause glomerulonephritis. Symptoms include swelling of the face or ankles, dark colored urine, or blood in the urine, or a decrease in urine production. Advise patients to report signs or symptoms of glomerulonephritis immediately. Monitoring of patients with renal dysfunction is recommended.
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with Renal Dysfunction.
The use of ibrutinib in patients with mild to moderate renal dysfunction (CrCl greater than 25 mL/min [as estimated by Cockcroft-Gault equation]) is not expected to influence on the exposure to ibrutinib. Caution and close monitoring are recommended in patients with severe renal dysfunction and in patients on dialysis as no data are available for these patients.
Sepsis
Moderate:
Neupogen
The filgrastim in Neupogen has a moderate potential interaction with Sepsis.
When filgrastim is used in septic patients, clinicians should be alerted to the theoretical possibility of adult respiratory distress syndrome, which may occur as a result of the influx of neutrophils at the site of inflammation. Monitoring of respiratory status may be appropriate in these patients.
Sickle Cell Anemia
Moderate:
Neupogen
The filgrastim in Neupogen has a moderate potential interaction with Sickle Cell Anemia.
Severe and sometimes fatal sickle cell crises have been reported in sickle cell disease patients receiving filgrastim products. Consider the potential risks and benefits prior to initiating therapy in these patients.
Smoking
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with Smoking.
Thrombocytopenia
Moderate:
Neupogen
The filgrastim in Neupogen has a moderate potential interaction with Thrombocytopenia.
Filgrastim may occasionally lower platelet counts. Thrombocytopenia has been reported, although most often the effects are mild or moderate and not associated with serious hemorrhagic sequelae. Nevertheless, therapy with filgrastim should be administered cautiously in patients with preexisting thrombocytopenia. Frequent blood counts are recommended, and the dosage of filgrastim adjusted if necessary.
Tumor Lysis Syndrome
Moderate:
ibrutinib
Ibrutinib has a moderate potential interaction with Tumor Lysis Syndrome.
Tumor lysis syndrome has been reported with ibrutinib therapy infrequently. The baseline risk (e.g., high tumor burden) should be assessed and appropriate precautions should be taken. Patients should be monitored closely and treated as appropriate.
