Drug interactions

Moderate drug interactions

Moderate:

oxytocin lactulose

Applies to: oxytocin and Evalose (lactulose)
Monitor

Bowel cleansing as well as overuse of certain laxatives may cause electrolyte loss and increase the risk of torsade de pointes ventricular arrhythmia in patients treated with drugs that prolong the QT interval. Electrolyte disturbances including hypokalemia and hypomagnesemia have been reported with laxative abuse and are known risk factors for torsade de pointes associated with QT interval prolongation.

Management

Patients treated with drugs that prolong the QT interval should exercise caution when self-medicating with laxatives. The recommended dosage and duration of use should not be exceeded. Patients treated with lactulose for more than six months should be monitored periodically for electrolyte imbalance. Patients should be advised to seek prompt medical attention if they experience symptoms that could indicate the occurrence of torsade de pointes such as dizziness, lightheadedness, fainting, palpitation, irregular heart rhythm, shortness of breath, or syncope.

Food/lifestyle interactions

No alcohol/food interactions were found. However, this does not necessarily mean no interactions exist. Always consult your healthcare provider.

Disease interactions

7 potential interactions Applies to: 7 conditions, including High Blood Pressure, Inflammatory Bowel Disease, Diabetes Mellitus, Fluid Retention, and Galactosemia.

Cerebral Vascular Disorder

Moderate:

oxytocin

Oxytocin has a moderate potential interaction with Cerebral Vascular Disorder.

Hypertensive events, subarachnoid hemorrhage, and arrhythmias have occurred during oxytocin therapy. Oxytocin possesses presser activity. Caution is advised in patients with uncontrolled hypertension or cerebrovascular dysfunction.

Diabetes Mellitus

Moderate:

Evalose

The lactulose in Evalose has a moderate potential interaction with Diabetes Mellitus.

Since lactulose solution contains galactose and lactose, it should be used cautiously in patients with diabetes.

Fluid Retention

Moderate:

oxytocin

Oxytocin has a moderate potential interaction with Fluid Retention.

Volume overload can occur during administration of oxytocin. Oxytocin possess antidiuretic properties. Consideration should, therefore, be given to the possibility of volume overload and water intoxication, particularly when oxytocin is administered continuously by infusion and the patient is receiving fluids by mouth.

Galactosemia

Major:

Evalose

The lactulose in Evalose has a major potential interaction with Galactosemia.

Since lactulose solution contains galactose, use is contraindicated in patients who require a low galactose diet.

Gastrointestinal Obstruction

Major:

Evalose

The lactulose in Evalose has a major potential interaction with Gastrointestinal Obstruction.

The use of laxatives is contraindicated in patients with intestinal obstruction disorders. Patients with intestinal obstruction disorders may need their underlying condition treated to correct the constipation. Some laxatives require reduction in the colon to their active form to be effective which may be a problem in patients with intestinal obstruction.

High Blood Pressure

Moderate:

oxytocin

Oxytocin has a moderate potential interaction with High Blood Pressure.

Hypertensive events, subarachnoid hemorrhage, and arrhythmias have occurred during oxytocin therapy. Oxytocin possesses presser activity. Caution is advised in patients with uncontrolled hypertension or cerebrovascular dysfunction.

Inflammatory Bowel Disease

Major:

Evalose

The lactulose in Evalose has a major potential interaction with Inflammatory Bowel Disease.

The use of laxatives is contraindicated in patients with inflammatory bowel disease. Patients with inflammatory bowel disease may experience colonic perforation with use of stimulant laxatives.

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
