Interactions between Entacapone and Myocard-DX
1 drug interaction and 11 additional warnings found for the following 2 drugs:
- entacapone
- Myocard-DX (dopamine)
Drug interactions
Moderate drug interactions
Moderate:
DOPamine entacapone
Applies to: Myocard-DX (dopamine) and entacapone
Monitor
COMT inhibitors affect the elimination of drugs metabolized by catechol-O-methyltransferase (COMT) and may potentiate their effects. Arrhythmias, increased heart rate, and blood pressure changes may occur. Entacapone has increased heart rates up to 80% when coadministered with isoproterenol and epinephrine. Ventricular tachycardia has also been reported.
Management
The manufacturer recommends that caution be exercised when these drugs are given together. Monitoring for changes in hemodynamic status and ECG is recommended.

Generally avoid
Alcohol may potentiate some of the pharmacologic effects of CNS-active agents. Use in combination may result in additive central nervous system depression and/or impairment of judgment, thinking, and psychomotor skills.
Management
Patients receiving CNS-active agents should be warned of this interaction and advised to avoid or limit consumption of alcohol. Ambulatory patients should be counseled to avoid hazardous activities requiring complete mental alertness and motor coordination until they know how these agents affect them, and to notify their physician if they experience excessive or prolonged CNS effects that interfere with their normal activities.

Moderate:
DOPamine Caffeine
Applies to: Myocard-DX (dopamine)
Monitor
Coadministration of two or more sympathomimetic or noradrenergic agents may increase the risk of nervousness, irritability, tachycardia, and/or increased blood pressure. Central nervous system (CNS) stimulants, particularly amphetamines, can potentiate the adrenergic response to vasopressors and other sympathomimetic and/or noradrenergic agents, including norepinephrine transporter inhibitors. Additive increases in blood pressure and heart rate may occur due to enhanced peripheral sympathetic activity.
Management
Caution is advised if two or more sympathomimetic or noradrenergic agents are coadministered. Pulse and blood pressure should be closely monitored. Consultation with individual product labeling, as well as relevant institutional protocols, may be advisable for further guidance.

Disease interactions
9 potential interactions Applies to: 9 conditions, including Asthma, Arrhythmia, Hypotension, Peripheral Arterial Disease, and Psychosis.
Arrhythmia
Major:
Myocard-DX
The DOPamine in Myocard-DX has a major potential interaction with Arrhythmia.
The use of dopamine is contraindicated in patients with uncorrected tachyarrhythmias or ventricular fibrillation. Dopamine exerts an agonistic action on the beta receptors of the heart which may lead to an increase in heart rate, atrial fibrillation, enhanced A-V conduction, ventricular ectopy, and arrhythmias.
Asthma
Major:
Myocard-DX
The DOPamine in Myocard-DX has a major potential interaction with Asthma.
Dopamine injection contains sodium metabisulfite, a sulfite that may cause anaphylactic symptoms and life-threatening or less severe asthmatic episodes in susceptible people, such as asthmatic patients.
Dehydration
Major:
Myocard-DX
The DOPamine in Myocard-DX has a major potential interaction with Dehydration.
The use of dopamine has been infrequently associated with significant hypotension especially in dehydrated patients secondary to beta-2 mediated vasodilation. Hypovolemia should be corrected before administering dopamine. Blood pressure and ECG should be monitored at regular intervals. Monitoring of cardiac output and pulmonary wedge pressure is recommended.
Hypotension
Moderate:
entacapone
Entacapone has a moderate potential interaction with Hypotension.
Dopamine agonists may impair the systemic regulation of blood pressure, with resultant orthostatic hypotension at any time, but especially during dose escalation. Additionally, patients with Parkinson's disease may have an impaired capacity to respond to an orthostatic challenge. For these reasons, patients with Parkinson's disease (or restless legs syndrome) who are being treated with dopaminergic agonists typically require careful monitoring for signs/symptoms of orthostatic hypotension, especially during dose escalation, and should be advised of this risk.
Ischemic Heart Disease
Major:
Myocard-DX
The DOPamine in Myocard-DX has a major potential interaction with Ischemic Heart Disease.
The use of dopamine may worsen ischemic heart disease. The stimulation of cardiac beta receptors may aggravate ischemic heart disease. Therapy with dopamine should be administered cautiously in patients with ischemic heart disease. Monitoring of ECG, blood pressure, urine flow, and cardiac output is recommended.
Liver Disease
Moderate:
entacapone
Entacapone has a moderate potential interaction with Liver Disease.
Entacapone should be used with caution in patients with hepatic impairment, as the AUC and Cmax of the drug approximately doubled in patients with documented liver disease compared to patients with normal liver function.
Peripheral Arterial Disease
Major:
Myocard-DX
The DOPamine in Myocard-DX has a major potential interaction with Peripheral Arterial Disease.
The vasoconstrictive actions of dopamine may aggravate occlusive vascular disease (atherosclerosis, arterial embolism, Raynaud's disease, cold injury, diabetic endarteritis, or Buerger's disease). Therapy with dopamine should be administered cautiously in patients with occlusive vascular disease. Patients should be monitored for decreased circulation to the extremities (change in color or temperature of the skin).
Pheochromocytoma
Major:
Myocard-DX
The DOPamine in Myocard-DX has a major potential interaction with Pheochromocytoma.
The use of dopamine is contraindicated in patients with pheochromocytoma. The catecholamine effects of dopamine may aggravate this condition.
Psychosis
Moderate:
entacapone
Entacapone has a moderate potential interaction with Psychosis.
Ordinarily, patients with major psychotic disorder should not be treated with dopaminergic antiparkinsonian agents, because of the risk of exacerbating psychosis. Hallucinations and psychotic-like behavior have been reported with dopaminergic medications. In addition, certain medications used to treat psychosis may exacerbate the symptoms of Parkinson's disease and may decrease the effectiveness of these drugs. The use of bromocriptine in patients with severe psychotic disorders is not recommended.
