Drug interactions
Moderate drug interactions
Moderate:
tadalafil dabrafenib
Applies to: finasteride / tadalafil and dabrafenib
Monitor
Coadministration with inducers of CYP450 3A4 may decrease the plasma concentrations of tadalafil, which is primarily metabolized by the isoenzyme. Rifampin (600 mg daily), a potent CYP450 3A4 inducer, has been reported to decrease tadalafil peak plasma concentration (Cmax) and systemic exposure (AUC) from a single 10 mg dose by 46% and 88%, respectively, compared to administration of the drug alone.
Management
Caution is advised if tadalafil is prescribed with CYP450 3A4 inducers. For the treatment of benign prostatic hyperplasia and erectile dysfunction, no dosage adjustment of tadalafil is warranted during coadministration with CYP450 3A4 inducers. However, the possibility of diminished pharmacologic effects of tadalafil should be considered. For the treatment of pulmonary arterial hypertension, concomitant use of tadalafil with patients on chronic potent CYP450 3A4 inducer therapy is not recommended. The manufacturer of the combination therapy macitentan-tadalafil (Opsynvi), also advises against their concomitant use.

Food/lifestyle interactions
Moderate food/lifestyle interactions
Moderate:
tadalafil Food/Lifestyle
Applies to: finasteride / tadalafil
Generally avoid
Coadministration with grapefruit juice is likely to increase the plasma concentrations of tadalafil, which is primarily metabolized by CYP450 3A4. However, the interaction has not been studied. The proposed mechanism is inhibition of CYP450 3A4-mediated first-pass metabolism in the gut wall by certain compounds present in grapefruit.
Monitor
Additive hypotensive effects may occur when phosphodiesterase-5 (PDE5) inhibitors such as tadalafil are used with alcohol, as both are mild systemic vasodilators. In clinical pharmacology studies, more subjects administered alcohol at a dose of 0.7 g/kg (equivalent to approximately 6 ounces of 80-proof vodka in an 80-kg male; consumed within 10 minutes in study subjects, providing blood alcohol levels of 0.08%) in combination with tadalafil 10 or 20 mg single doses had clinically significant decreases in blood pressure than with alcohol alone. There were reports of postural dizziness, and orthostatic hypotension was observed in some. When tadalafil 20 mg was administered with alcohol at a lower dose of 0.6 g/kg (equivalent to approximately 4 ounces of 80-proof vodka in an 80-kg male), orthostatic hypotension was not observed, dizziness occurred with similar frequency relative to alcohol alone, and the hypotensive effects of alcohol were not potentiated. Neither tadalafil nor alcohol affected the plasma concentrations of the other.
Management
Caution is recommended with concurrent consumption of large amounts of alcohol in patients taking tadalafil as it may increase the potential for orthostatic signs and symptoms, such as increase in heart rate, decrease in standing blood pressure, dizziness, and headache. It may also be appropriate to avoid consuming large amounts of grapefruit juice.

In clinical trials, patients with a history of diabetes who received dabrafenib required more intensive hypoglycemic therapy. Care should be exercised when using this drug in patients with preexisting diabetes or hyperglycemia. Serum glucose levels should be monitored at the start of therapy and as clinically indicated in these patients. Antihyperglycemic medications should be started or optimized as clinically indicated.
Fever
Moderate:

Dabrafenib has a moderate potential interaction with Fever.
Serious febrile reactions and fever of any severity complicated by rigors/chills, hypotension, dehydration, and/or renal failure have been observed with the use of dabrafenib. The incidence and severity of pyrexia are increased when this drug is administered in combination with trametinib compared with dabrafenib as a single agent. It is recommended to withhold therapy for temperature of 38C (100.4F) or higher; in case of recurrence, therapy can be interrupted at first symptom of pyrexia. Patients should be evaluated for signs/symptoms of infection, and it is recommended to monitor serum creatinine and other evidence of renal function during and after severe pyrexia. Antipyretics should be administered as secondary prophylaxis when resuming therapy for patients with prior episode of severe febrile reaction or fever associated with complications. Corticosteroids (e.g., prednisone 10 mg/day) should be administered for at least 5 days for second or subsequent pyrexia if temperature does not return to baseline within 3 days of pyrexia onset, or for pyrexia associated with complications, and there is no evidence of active infection. Upon resolution of pyrexia, dose reduction and/or permanent therapy discontinuation may be required.

Prolonged erection greater than 4 hours and priapism (painful erections greater than 6 hours) have been reported during treatment with phosphodiesterase-5 (PDE 5) inhibitors. Priapism may result in penile tissue damage and permanent loss of potency if not treated promptly. These agents should be used cautiously in patients with conditions that may predispose them to priapism such as sickle cell anemia, multiple myeloma, or leukemia, and those with anatomical deformation of the penis (such as angulation, cavernosal fibrosis, or Peyronie's disease). If an erection persists longer than 4 hours, the patient should seek immediate medical assistance.

Tadalafil for the treatment of erectile dysfunction (ED) should be used with caution and only if the benefit outweighs the risk in patients with a history of non-arteritic anterior ischemic optic neuropathy (NAION) or with retinitis pigmentosa. Use of tadalafil for the treatment of pulmonary arterial hypertension is not recommended in patients with retinitis pigmentosa. Advise patients to seek immediate medical attention in the event of a sudden loss of vision in one or both eyes while taking tadalafil; patients taking tadalafil for ED should immediately stop treatment.
