Food/lifestyle interactions
Moderate food/lifestyle interactions
Moderate:
tepotinib Food/Lifestyle
Applies to: Tepmetko (tepotinib)
Adjust dosing interval
Food enhances the oral bioavailability of tepotinib. When tepotinib was administered after a high-fat, high-calorie meal (approximately 800 to 1000 calories; 150 calories from protein, 250 calories from carbohydrate, 500 to 600 calories from fat), tepotinib peak plasma concentration (Cmax) and systemic exposure (AUC) increased by 2-fold and 1.6-fold, respectively, compared to administration under fasted conditions.
Management
Tepotinib should be administered with food at approximately the same time each day.

Disease interactions
7 potential interactions Applies to: 7 conditions, including Depression, Liver Disease, Infection, Immunodeficiency, and Renal Dysfunction.
Depression
Moderate:
Benlysta
The belimumab in Benlysta has a moderate potential interaction with Depression.
In clinical trials, depression and suicidality were reported in patients receiving belimumab. Before treatment with belimumab, the risk of depression and suicide should be assessed, considering the patient's medical history and current psychiatric status; patients should continue to be monitored during therapy. Patients receiving belimumab (and caregivers, if applicable) should be instructed to contact their health care provider if they experience new or worsening depression, suicidal thoughts or behaviors, or other mood changes. The risk and benefit of continued therapy should be considered for patients who develop such symptoms.
Immunodeficiency
Moderate:
Benlysta
The belimumab in Benlysta has a moderate potential interaction with Immunodeficiency.
Cases of John Cunningham virus (JC virus)-associated progressive multifocal leukoencephalopathy (PML) resulting in neurological deficits (including fatal cases) have been reported in patients with systemic lupus erythematosus receiving immunosuppressants (including belimumab). Risk factors for PML include treatment with immunosuppressant therapies and impairment of immune function. The diagnosis of PML should be considered in any patient presenting with new-onset or deteriorating neurological signs/symptoms; a neurologist or other appropriate specialist should be consulted as clinically indicated. If PML is confirmed, belimumab should be discontinued.
Infection
Moderate:
Benlysta
The belimumab in Benlysta has a moderate potential interaction with Infection.
Serious and sometimes fatal infections have been reported in patients receiving immunosuppressive agents (including belimumab). Overall, while the incidence of serious infections in controlled trials was similar between belimumab and placebo, fatal infection occurred more often with belimumab. The risk and benefit should be considered before starting treatment with belimumab in patients with severe or chronic infections. Interruption of therapy should be considered in patients who develop a new infection while receiving belimumab and these patients should be monitored closely.
Liver Disease
Moderate:
Tepmetko
The tepotinib in Tepmetko has a moderate potential interaction with Liver Disease.
The use of tepotinib may cause hepatotoxicity, sometimes resulting in fatalities. No dosage modification is recommended in patients with mild (Child-Pugh A) or moderate (Child-Pugh B) liver dysfunction. Care should be exercised when using tepotinib in patients with severe liver dysfunction (Child-Pugh C) as its pharmacokinetics and safety have not been studied in this patient population. It is recommended to monitor liver function tests (including ALT, AST, and total bilirubin) prior to the start of therapy, every 2 weeks during the first 3 months of therapy, then once a month or as clinically indicated, with more frequent testing in patients who develop increased transaminases or bilirubin. Tepotinib should be withheld, the dose should be reduced, or therapy should be permanently discontinued based on the severity of the adverse reaction.
Psychiatric Disorders
Moderate:
Benlysta
The belimumab in Benlysta has a moderate potential interaction with Psychiatric Disorders.
Pulmonary Impairment
Moderate:
Tepmetko
The tepotinib in Tepmetko has a moderate potential interaction with Pulmonary Impairment.
The use of tepotinib may cause interstitial lung disease (ILD)/pneumonitis, sometimes resulting in fatalities. Care should be exercised when using this drug in patients with preexisting pulmonary impairment. Tepotinib should be immediately withheld in patients with suspected ILD/pneumonitis and permanently discontinued if no other potential causes of ILD/pneumonitis are identified. Patients should be monitored for new/worsening pulmonary symptoms indicative of ILD/pneumonitis.
Renal Dysfunction
Moderate:
Tepmetko
The tepotinib in Tepmetko has a moderate potential interaction with Renal Dysfunction.
Care should be exercised when using tepotinib in patients with severe renal dysfunction (CrCl less than 30 mL/min) as the recommended dosage has not been established in this patient population. No dosage adjustment is recommended in patients with mild or moderate renal dysfunction (CrCl 30 to 89 mL/min).

Drug Interaction Classification
| Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit. | |
| Moderately clinically significant. Usually avoid combinations; use it only under special circumstances. | |
| Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan. | |
| No interaction information available. |
