Usual Adult Dose for Congenital Adrenal Hyperplasia

Initial dose: 100 mg orally twice a day, with the morning and evening meals

Use: Adjunctive treatment to glucocorticoid replacement to control androgens in patients with congenital adrenal hyperplasia (CAH)

Usual Pediatric Dose for Congenital Adrenal Hyperplasia

4 years and older:

• Weight 10 kg to less than 20 kg: 25 mg orally twice daily
• Weight 20 kg to less than 55 kg: 50 mg orally twice daily
• Weight at least 55 kg: 100 mg orally twice daily

Comment:

• Take with the morning and evening meals.

Use: Adjunctive treatment to glucocorticoid replacement for the control of androgens in patients with congenital adrenal hyperplasia (CAH)

Renal Dose Adjustments

Not recommended in severe renal impairment or end-stage renal disease.

Dosage Modifications for Concomitant Use with Strong CYP450 3A4 Inducers:

• Adult: Increase to 200 mg orally twice daily, with the morning and evening meals
• Pediatric:
• Weight 10 kg to less than 20 kg: 50 mg orally twice daily, with the morning and evening meals
• Weight 20 kg to less than 55 kg: 100 mg orally twice daily, with the morning and evening meals
• Weight at least 55 kg: 200 mg orally twice daily, with the morning and evening meals

Dosage Modifications for Concomitant Use with Moderate CYP450 3A4 Inducers:

• Adult: Increase to 100 mg orally with the morning meal and 200 mg with the evening meal
• Pediatric:
• Weight 10 kg to less than 20 kg: 25 mg orally with the morning meal and 50 mg orally with the evening meal
• Weight 20 kg to less than 55 kg: 50 mg orally with the morning meal and 100 mg orally with the evening meal
• Weight at least 55 kg: 100 mg orally with the morning meal and 200 mg orally with the evening meal

CONTRAINDICATIONS:

• Hypersensitivity to the active component or any of the ingredients

Safety and efficacy have not been established in patients younger than 4 years.

Storage requirements:

• Capsules:
• Store at 15C to 25C (59F to 77F).
• Do not freeze.

• Solution:
• Dispense and store in original container in an upright position.
• Keep unopened bottles refrigerated at 2C to 8C (36F to 46F).
• Once opened, keep bottles refrigerated at 2C to 8C (36F to 46F) or at room temperature (15C to 25C [59F to 77F]). Discard unused portion 30 days after opening.
• Do not freeze.
