Usual Adult Dose for COVID-19 Prophylaxis
Irrespective of COVID-19 vaccination status: 0.5 mL IM as a single dose
- If previously vaccinated with any COVID-19 vaccine: 0.5 mL IM as a single dose, administered at least 2 months after the last dose of COVID-19 vaccine
Comments:
- This vaccine is approved for use in adults who are 18 through 64 years of age with at least 1 underlying condition that places them at high risk for severe outcomes from COVID-19.
Use: For active immunization to prevent COVID-19 caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)

- This vaccine is approved for use in patients who are 6 months through 17 years of age with at least 1 underlying condition that places them at high risk for severe outcomes from COVID-19.
- This formulation is intended for patients 12 years and older.

Precautions
CONTRAINDICATIONS:
- Known history of severe allergic reaction (e.g., anaphylaxis) to the active component or any of the ingredients
- Severe allergic reaction (e.g., anaphylaxis) after a previous dose of a Moderna COVID-19 vaccine
Safety and efficacy of this vaccine have not been established in patients younger than 6 months.
- This formulation is intended for patients 12 years and older.

Administration advice:
- Before administration, verify that the label on the prefilled syringe shows the correct formula for the current season.
- This vaccine is a white to off-white suspension that may contain white or translucent product-related particulates; do not administer if vaccine is discolored or contains other particulate matter. Do not shake.
- Administer IM.
Storage requirements:
- Store frozen between -50C to -15C (-58F to 5F).
- During storage: Minimize exposure to room light; avoid exposure to direct sunlight and UV light.
- After thawing: May store refrigerated between 2C to 8C (36F to 46F) for up to 60 days or until the expiration date printed on the carton, whichever comes first OR may store between 8C to 25C (46F to 77F) for up to 12 hours
- Do not refreeze after thawing.
- Thawed syringes can be handled in room light conditions.
- Transportation of thawed syringes at 2C to 8C (36F to 46F): Can transport at 2C to 8C (36F to 46F) in shipping containers qualified to maintain this temperature
- Once thawed and transported at 2C to 8C (36F to 46F): Do not refreeze, and store at 2C to 8C (36F to 46F) until use.
Reconstitution/preparation techniques:
- If prefilled syringes are frozen, thaw before use as follows:
- Carton of 10 syringes: Thaw in refrigerator (2C to 8C [36F to 46F]) for 2 hours and 40 minutes OR thaw at room temperature (15C to 25C [59F to 77F]) for 1 hour and 20 minutes.
- Carton of 2 syringes or 1 syringe (removed from carton): Thaw in refrigerator for 1 hour and 40 minutes OR thaw at room temperature for 40 minutes.

- This formulation is intended for adults and adolescents (12 years and older).
- Appropriate medical treatment must be immediately available to manage potential anaphylactic reactions after administration of this vaccine.
