The use of thrombin inhibitors is contraindicated in the presence of active major bleeding. Additionally, these drugs should be used with caution in patients with disease states associated with an increased risk of bleeding.
Bivalirudin (applies to Angiomax) renal impairment
Moderate Potential Hazard, Moderate plausibility. Applicable conditions: Renal Dysfunction
Clinical studies showed that the clearance of bivalirudin is reduced approximately 20% with moderate and severe renal impairment and is reduced about 80% in dialysis-dependent patients. The infusion dose of bivalirudin may need to be reduced, and anticoagulant status monitored in patients with renal impairment.
