Dalteparin - Why cap the dose at 18,000 units? Some centers in UK cap at 25,000 units?

Agree not to exceed 200 iu/kg. even the 2009 chest study capped at 18000 but did not explain why they chose 18000 and in fact the policy in central uk nhs hospitals is 25,000 as recent as 2012.
