Major drug interactions

Major:

tildrakizumab ublituximab

Applies to: tildrakizumab and ublituximab

Using ublituximab together with tildrakizumab may increase the risk of serious infections. Talk to your doctor if you have any questions or concerns. Of particular concern is an infection known as progressive multifocal leukoencephalopathy (PML), which is a rare but serious viral infection of the brain that may lead to disability and death. Your doctor may already be aware of the risks but has determined that this is the best course of treatment for you and has taken appropriate precautions and is monitoring you closely for any potential complications. Let your doctor know if you develop signs and symptoms of infection such as fever, chills, sore throat, shortness of breath, blood in phlegm, weight loss; yellow skin, severe fatigue, diarrhea, vomiting, body aches; and pain or burning during urination. Also, seek immediate medical attention if you experience progressive weakness on one side of the body, clumsiness of limbs, disturbance of vision, confusion, or changes in thinking, memory, and personality, as these may be early symptoms of PML. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Disease interactions

10 potential interactions Applies to: 9 conditions, including Liver Disease, Infection, Tuberculosis, Active, Tuberculosis, Latent, and Immunodeficiency.

History, Tuberculosis

Major:

tildrakizumab

Tildrakizumab has a major potential interaction with History, Tuberculosis.

Do not administer tildrakizumab to patients with active TB infection. It is recommended to assess patients for tuberculosis (TB) infection before starting treatment with this agent. Consider anti-TB therapy prior to initiation of tildrakizumab in patients with latent TB, a past history of latent or active TB in whom an adequate course of treatment cannot be confirmed. Monitor patients for signs and symptoms of active TB during and after treatment with tildrakizumab.

Immunodeficiency

Moderate:

ublituximab

Ublituximab has a moderate potential interaction with Immunodeficiency.

Ublituximab has been associated with serious, including life-threatening or fatal, bacterial and viral infections. Delay treatment in patients with active infection until resolution. Monitor the levels of quantitative serum immunoglobulins during treatment, especially in patients with opportunistic or recurrent infections, and after discontinuation of therapy until B-cell repletion. For patients with low serum immunoglobulins, consult immunology experts before initiating therapy. Consider treatment discontinuation if a patient with low immunoglobulins develops a serious opportunistic infection or recurrent infections, or if prolonged hypogammaglobulinemia requires therapy with IV immunoglobulins.

Infection

Moderate:

tildrakizumab

Tildrakizumab has a moderate potential interaction with Infection.

The use of tildrakizumab may increase the risk of infection. Treatment with tildrakizumab should not be started in patients with active infection until the infection resolves or is adequately treated. Consider the risks and benefits prior to using this agent in patients with chronic infection or a history of recurrent infection. It is recommended to monitor the patient closely and consider discontinuation of tildrakizumab if a patient develops a clinically important or serious infection or is not responding to standard therapy.

Moderate:

ublituximab

Ublituximab has a moderate potential interaction with Infection.

Infectious Hepatitis

Major:

ublituximab

Ublituximab has a major potential interaction with Infectious Hepatitis.

Use of ublituximab is contraindicated in patients with active hepatitis b virus (HBV) infection. Reactivation of HBV has been reported with use of ublituximab. Perform HBV screening in all patients prior to therapy. Consult a liver disease expert in patients who are negative for surface antigen (HBsAg) and positive for HB core antibody (HBcAb+), or are carriers of HBV (HBcAb+).

Liver Disease

Moderate:

ublituximab

Ublituximab has a moderate potential interaction with Liver Disease.

Ublituximab may cause liver injury. Patients with ALT/AST greater than 3 times the upper limit of normal (3 x ULN) and total bilirubin greater than 2 x ULN are at risk for severe drug-induced liver injury. The effect of moderate to severe liver dysfunction on the pharmacokinetics of ublituximab is unknown. Prior to therapy with ublituximab, assess ALT, AST, alkaline phosphatase, and bilirubin levels. Monitor for signs and symptoms of any drug-induced liver injury during treatment; discontinue therapy if identified.

Renal Dysfunction

Moderate:

ublituximab

Ublituximab has a moderate potential interaction with Renal Dysfunction.

The effect of moderate to severe renal dysfunction on the pharmacokinetics of ublituximab is unknown.

Tuberculosis, Active

Major:

tildrakizumab

Tildrakizumab has a major potential interaction with Tuberculosis, Active.

Tuberculosis, Latent

Major:

tildrakizumab

Tildrakizumab has a major potential interaction with Tuberculosis, Latent.

Vaccination

Moderate:

tildrakizumab

Tildrakizumab has a moderate potential interaction with Vaccination.

It is recommended to avoid the use of live vaccines in patients treated with tildrakizumab. Before starting therapy with tildrakizumab, consider completion of all age appropriate immunizations according to current immunization guidelines. Care should be exercised as no data are available on the response to live or inactive vaccines.

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
